What the Beauty Spa Client Intake Form Is

The Beauty Spa Client Intake Form is a standardized client-facing document used by spas and salons to collect essential personal, medical, and service-preference information before treatments. It records contact details, emergency contact, medical history, allergies, medications, skin conditions, pregnancy status, prior procedures, and consent for treatments and photography. Completed intake forms support treatment planning, liability risk reduction, and continuity of care. When stored with appropriate consent and security controls, they become part of the client record and help meet state, HIPAA, and professional requirements where applicable.

Why a Complete Intake Form Matters for Spas

Practical Tips for Accurate and Efficient Intake

Apply these practices to reduce errors, speed processing, and protect client safety and privacy.

Pre-Appointment Intake
Send the intake form digitally before the appointment to let clients complete it at home; this reduces in-clinic time and allows staff to flag issues in advance.
Use Conditional Fields
Configure fields to appear based on answers (e.g., pregnancy question shows additional screening), which decreases irrelevant questions and prevents overlooked contraindications.
Verify Identity
Match the name and DOB on the form to a photo ID for treatments with higher risk, and record verification steps within the client record for auditability.
Maintain Audit Trails
Store signed PDFs and an immutable audit trail (timestamps, IP, signer identity). Keep export processes documented for compliance reviews or incident response.

  • This workflow reduced in-person intake time and provided defensible records if follow-up or insurance inquiries arose.
