By remitting payment, Client acknowledges that the amounts invoiced reflect premiums, administrative fees, and any adjustments authorized for the billing period specified above. Payment is due in full on or before the Due Date shown. Unless otherwise agreed in writing, amounts unpaid after the Due Date shall accrue interest at the lesser of 1.5% per month (18% per annum) or the maximum rate permitted by applicable law. Client shall be liable for all collection costs, including reasonable attorneys' fees and court costs, incurred to collect overdue amounts.

Disputes must be submitted in writing within 15 days of the Invoice Date and must identify the disputed amount and the grounds for dispute. Failure to provide timely written notice shall be deemed a waiver of any dispute. Adjustments or credits will be issued only in accordance with written agreement or documented policy.

This invoice does not alter the underlying benefit plan documents. Suspension or termination of services for nonpayment shall be governed by the service agreement between the parties. Client represents that it has authority to authorize payment on behalf of the billed entity.
