By signing below, Payee / Beneficiary hereby authorizes Payor to direct payment of benefits described in this form to the bank account or remittance address identified above. This authorization constitutes a continuous assignment of future benefit payments as identified, until revoked in writing. Payee represents that the information provided is true, complete, and that the person or entity named as Payee is entitled to receive the payments described.

Payor is authorized to verify account information and to contact financial institutions or third parties as necessary to effectuate deposits. Payee consents to release of payment-related information to banking institutions and to any third party acting on Payor's behalf for the purpose of payment processing and verification.

Payee agrees to indemnify and hold harmless Payor and the receiving financial institution from any loss, claim, damage, or expense (including reasonable attorneys' fees) arising from errors in account information, unauthorized account entries resulting from Payee-provided instructions, or from Payee's failure to timely notify Payor of changes or errors. If Payee submits incorrect account information resulting in misdirected funds, Payee is responsible for resolving the matter and returning funds incorrectly paid.

This authorization remains in effect until Payee provides written notice of revocation to Payor at the address above and Payor has had a reasonable period (not less than ten business days) to act on such notice. Revocation does not relieve Payee of obligations for payments already authorized and processed prior to effective revocation.
