What the Birthstone Selection Form Is and captures

Birthstone Selection Form defines customer choices and specifications for a custom birthstone jewelry order or symbolic gift. It records the customer's name, contact details, selected birthstone(s), gemstone grade or color variant, setting type, metal preference, ring/resizing details, engraving instructions, and delivery preferences. The form can also capture approval for alternatives if a chosen stone is unavailable, and records payment authorization and estimated lead time. When used with a compliant eSignature workflow, it provides an auditable record of customer consent and order acceptance.

Why a standard form matters for order accuracy

A clear Birthstone Selection Form reduces order errors, documents customer approvals, and establishes specifications for production. It standardizes choices for fulfilment teams, supports accurate pricing and inventory checks, and creates a verifiable record when combined with electronic signatures under ESIGN and UETA.
