What the Brokerage Client Questionnaire Is and When It Applies

A Brokerage Client Questionnaire is a standardized intake form used by broker-dealers and investment advisors to collect client identity, financial profile, investment objectives, suitability data, and compliance disclosures required for account opening and ongoing relationship management. It consolidates KYC (know your customer) details, tax and regulatory data, risk tolerance, and information needed to comply with anti-money laundering checks, investor accreditation verification, and broker-dealer recordkeeping. Completed questionnaires form part of the client file used for onboarding, suitability determinations, account maintenance, and regulatory audits.

Why a Complete Questionnaire Matters for Compliance and Service

A thorough Brokerage Client Questionnaire reduces regulatory risk, documents suitability decisions, and accelerates account setup. Accurate answers ensure correct product matches and create an auditable record for FINRA and SEC reviews.

Why a Complete Questionnaire Matters for Compliance and Service

Who Prepares and Reviews the Questionnaire

Roles vary by firm size and structure, but the common objective is consistent: accurate information to satisfy regulatory obligations and support investment recommendations.

  • Registered representatives and advisors complete client-facing sections and collect identity documents during account opening.
  • Compliance officers review responses for AML, sanctions screening, and investor accreditation verification before account activation.
  • Operations teams maintain the completed questionnaire in the account record and use it for ongoing monitoring and periodic updates.

Core Sections Included in a Professional Brokerage Client Questionnaire

A well-structured questionnaire groups related data and minimizes ambiguous answers to support clear suitability findings and recordkeeping.

Client Identity

Full legal name, date of birth, government ID details, Social Security number or TIN, and residential address for identity verification and tax reporting.

Contact Information

Primary phone, email, and mailing address plus preferred communication method and account delivery instructions for notifications and statements.

Financial Profile

Income range, net worth (with or without primary residence), liquidity needs, and employment or business details to assess suitability.

Investment Objectives

Specified goals such as growth, income, preservation of capital, time horizon, and constraints that determine recommended products.

Risk Tolerance

Quantified tolerance to volatility and loss, often captured via standardized scoring or scenario questions used for suitability.

Regulatory Disclosures

Declarations for accreditation, political exposure, sanctions, tax residency, and consent to electronic delivery where applicable.
