What the Business AFD Template Is

The Business AFD Template is a standardized affidavit-style form used to disclose financial information, ownership interests, or funding details for a business matter. It collects structured data about parties, transactions, and material relationships and is often attached to contracts, corporate filings, procurement records, or internal compliance reviews. The template is designed for consistent presentation of facts under oath or penalty of perjury, and to support decision-making, audit trails, and legal review while remaining adaptable to state- or industry-specific requirements.

Why a Clear Business AFD Template Matters

A complete, well-organized Business AFD Template reduces ambiguity, supports regulatory compliance, and creates an auditable record of financial disclosures for internal and external stakeholders.

Why a Clear Business AFD Template Matters

Who Typically Completes a Business AFD Template

Proper signatory selection and accurate entries reduce downstream review time and legal exposure.

  • Company officers and authorized representatives completing disclosures during M&A, board reviews, or vendor onboarding.
  • Legal and compliance teams verifying ownership, material relationships, or funding sources for due diligence.
  • Procurement or contract administrators collecting sworn financial statements before award or execution.

Essential Elements of a Professional Business AFD Template

A thorough template groups identification, factual disclosures, supporting attachments, certification language, signature blocks, and routing instructions to ensure clarity and enforceability.

Identification

Full party names, business legal entity type, EIN or tax ID, and contact details to ensure the affidavit is attributable to a specific legal person or entity.

Disclosure List

A clear, itemized section for financial interests, loans, guarantees, ownership percentages, material contracts, and related-party transactions with dates and amounts where applicable.

Supporting Documents

Attachment placeholders for bank statements, shareholder ledgers, loan agreements, or other evidence; indicate required file types and page numbering for consistency.

Certification Clause

Standard sworn-language that sets the statement under penalty of perjury and identifies jurisdictional venue and governing law for interpretation.

Signature Block

Designated signer fields for printed name, title, date, and witness/notary sections if required; include role-based signer instructions to prevent improper signers.

Routing Instructions

Defined internal workflow fields (reviewers, approvers, final custodian) and expected response windows to streamline processing and archival.

Common Mistakes to Avoid

  • Entering informal or abbreviated names that do not match legal records, which can delay verification and create ambiguity.
  • Omitting attachments or failing to label exhibits, causing reviewers to miss supporting evidence during due diligence or audit.
  • Using inconsistent date formats across fields, which can create disputes about the effective date or time-sensitive obligations.
  • Allowing an unauthorized person to sign; verify authority and corporate delegation before accepting the executed affidavit.

Penalties and Legal Risks of an Incorrect Template

Information Return Penalties: IRC §6721: $60–$660+ per incorrect information return
