What a Business AMI Sheet Is and when it's used

A Business AMI Sheet is a standardized information and authorization form used to collect core business details, account and billing information, authorized signatories, and compliance attestations required to open or maintain a business account or service relationship. Organizations use it to centralize contact data, payment terms, tax identifiers, required certifications, and document attachments so onboarding, billing, and audit trails are consistent and auditable across teams and vendors.

Why a consistent Business AMI Sheet matters

Standardizing the Business AMI Sheet reduces rework, lowers onboarding time, and creates a clear audit trail for approvals and regulatory review without depending on email threads or fragmented records.

Why a consistent Business AMI Sheet matters

Who typically prepares and signs a Business AMI Sheet

Different teams interact with this form depending on the workflow stage; prepare it with the final recipient in mind.

  • Accounts payable and receivable teams responsible for billing setup and vendor onboarding.
  • Sales or account managers who collect customer data and confirm commercial terms.
  • Legal or compliance officers who confirm signatory authority and required regulatory attestations.

Keep the form accessible to cross-functional reviewers to prevent duplicate requests and speed approvals.

Essential sections that belong on a professional Business AMI Sheet

A well-structured AMI Sheet groups related data into predictable sections so users supply the right evidence and reviewers can verify identity, authority, and compliance quickly.

Business Identification

Legal entity name, DBA, business type, and state of formation. Include EIN or tax identifier to match tax reporting records and prevent backup withholding.

Primary Contacts

Names, titles, phone numbers, and emails for billing, operations, and legal contacts so notifications and escalation paths are clear.

Authorized Signatory

Name, title, signer authority basis (board resolution, corporate officer), and a statement confirming authority to bind the entity.

Payment & Billing Details

Preferred billing method, payment terms, remittance address, ACH or wire details, and any billing contact preferences or invoicing constraints.

Compliance & Attachments

Required supporting documents such as W-9, certificates of insurance, licenses, or HIPAA business associate addenda where applicable.

Audit Trail & Versioning

Record of who completed or updated the sheet, timestamps, and retained copies of prior versions to establish a defensible change history.

Step-by-step: completing a Business AMI Sheet

  • 01
Collect documents: Assemble W-9, insurance, licenses before starting.
  • 02
Populate core fields: Enter legal name, EIN, addresses, and contact info.
  • 03
Confirm signer authority: Attach resolution or proof of authority if needed.
  • 04
Sign and save: Sign, date, and store the signed copy with attachments.

How to configure the online workflow for this sheet

Set up a predictable routing workflow to ensure the correct approvers and reviewers see the sheet in the proper order.

Field Configuration
Authentication Method Email link, SMS code, or knowledge-based authentication
Field Validation Require EIN pattern, ZIP code, and date format checks
Conditional Logic Reveal insurance fields only for certain industries
Routing Order Collect signatory signature then legal then finance

Technical and delivery requirements for digital completion

Ensure the chosen platform supports required file formats, audit logs, and the authentication method your compliance team requires.

  • File formats: PDF, DOCX, or scanned image
  • Integrations: Salesforce, NetSuite, Google Workspace
  • Security: TLS and AES-256 encryption

Confirm the platform produces a tamper-evident signed PDF, records a detailed audit trail, and can export signed copies to your document management system for retention.

Where to send and how the Business AMI Sheet moves

A clear routing path prevents delays; list primary recipients and the expected delivery method for each step.

  • Vendor onboarding: Send to vendor onboarding email or portal
  • Finance: Deliver signed copy to accounts payable
  • Legal review: Route to counsel when signatory authority is unclear
  • Records: Archive in company DMS for retention

Common preparation errors to avoid

  • Using a trade name instead of the company’s legal name, causing mismatches with tax and bank records that block payments and tax reporting.
  • Failing to attach required supporting documents such as a W-9 or insurance certificate, which delays onboarding and can trigger holdbacks.
  • Submitting an incorrect or improperly formatted EIN or SSN, which can require backup withholding or subsequent corrections with the IRS.
  • Omitting signer authority details or uploading an expired authorization, leading compliance teams to request additional proof and delay contract execution.

Risks and potential penalties from incorrect or incomplete sheets

Backup withholding: 24% if missing or incorrect TIN
Tax filing penalties: Per-form penalties under IRC §6721
Contract disputes: Ambiguous signatory authority can void obligations
Regulatory fines: Industry-specific penalties for noncompliance
Payment delays: Missing details block invoice processing
Data breach exposure: Insufficient security can trigger notice obligations
