How organizations apply the form in practice

Representative examples show how organizations adapted the Business App Title for online execution, compliance, and integration with existing systems.

Martin Properties

Tim Martin, founder of Martin Properties, used the Business App Title to move lease and vendor processes entirely online, including mobile and offline capability.

  • Mobile signing improved turnaround times.
  • He reported full compliance and efficient returns with an audit trail that supported recordkeeping and occasional offline execution when agents were on site; the digital workflow reduced multi-party coordination time substantially.

Fertility Centers of Illinois

John Butler, founder at Fertility Centers of Illinois, integrated the form into intake and consent workflows and cited API reliability and responsive support.

  • API integration eased case management.
  • The organization used the platform to automate record retention and ensure audit trails for consent records, improving administrative accuracy and making regulatory responses more straightforward for clinical and billing requests.

Common preparation errors to avoid

  • Entering an incorrect EIN or TIN is common; mismatches between submitted tax identifiers and IRS records can trigger backup withholding, payment delays, and additional verification requests.
  • Omitting required attachments such as formation documents or proof of authority often results in application rejection and repeat submissions, which lengthen processing time and add administrative cost.
  • Using inconsistent legal names—abbreviations, nicknames, or omission of suffixes—can cause vendor setup failures or require re-execution of documents under the correct entity name.
  • Failing to confirm signer authority or to attach a corporate resolution or power of attorney creates legal uncertainty and increases the risk that an agreement will be challenged.

Potential penalties and legal risks

Backup Withholding: 24% rate for incorrect TIN
Application Denial: Incomplete forms delay approval
Late Filing Penalties: Per-form fines under IRC §6721
Invalid Signature: May void transaction or agreement
Missing Notary: May invalidate notarized acknowledgements
Data Breach: Regulatory fines and remediation costs
