The Business Banking Detail Form collects a company's bank account information, routing numbers, and authorization to initiate ACH transfers or direct debits for payroll, supplier payments, or deposits. Typical uses include vendor setup, merchant onboarding, payroll direct deposit enrollment, and recurring payment authorizations. Common Mistakes to Avoid

  • Entering a routing or account number with transposed digits leading to misrouted or returned payments and reconciliation delays.
  • Accepting a scanned signature without verifying signer authority, which can increase fraud and expose the organization to unauthorized debits.
  • Failing to attach a voided check or bank letter, making the account unverifiable and delaying bank onboarding by several days.
  • Not recording the effective date or termination instructions, causing uncertainty about when payments should start or stop.

Penalties, Risks, and Compliance Considerations

Payment Returns: Reprocessing fees and delayed vendor payment
Backup Withholding: Incorrect TIN may trigger 24% withholding
Fraud Exposure: Unauthorized account changes can cause loss
Regulatory Audit: Insufficient records can fail inspections
Contract Disputes: Invalid authorization risks legal claims
Operational Delay: Missing verification stalls onboarding
