What the Business Bribery TBT Form Is

The Business Bribery TBT Form is an internal compliance and reporting template used to document allegations, incidents, or investigations involving suspected bribery or improper payments. It standardizes the information collected about parties, dates, amounts, and supporting evidence so compliance, legal, and investigation teams can triage, assign responsibility, and preserve an auditable record. The form is intended for corporate use, supports attachment of exhibits and evidence, and can be completed and retained electronically to maintain chain-of-custody and event timestamps when supported by an audit trail.

Why a Structured Bribery Report Matters

A consistent Business Bribery TBT Form reduces investigatory delays, preserves essential facts for regulatory review, and creates a defensible audit trail. Clear inputs help determine whether to escalate to legal, compliance, or law enforcement, and support required recordkeeping under corporate anti-corruption policies and external statutes.

Why a Structured Bribery Report Matters

Who Typically Prepares and Reviews This Form

Use this form when compliance officers, internal audit, legal teams, or managers identify potential bribery or improper payment activity.

  • Compliance teams: document intake, preliminary assessment, and escalation notes.
  • Legal counsel: review for privilege, regulatory obligations, and reporting requirements.
  • Managers and HR: supply witness names, immediate containment actions, and local context.

Completed reports are routed to compliance and legal reviewers, and may be retained in the corporate records management system for the retention period applicable to investigations and legal matters.

Who Signs and Approves the Form

Compliance Officer

Typically completes the assessment section and certifies that the report meets internal policy thresholds. The officer documents steps taken, assigns investigators, and notes required external reporting obligations when applicable.

General Counsel

Provides legal review, confirms privilege status, advises on regulatory notifications, and signs off on decisions to report to external authorities or to pursue remediation and disciplinary action.

Essential Components of a Professional Bribery TBT Form

A robust form captures identity, chronology, financial detail, corroborating evidence, immediate containment, and routing metadata. Each component should be discrete, searchable, and structured to enable triage and follow-up.

Reporter Details

Contact information, role, department, and whether the reporter requests confidentiality or whistleblower protections; this enables follow-up and preserves reporter rights.

Incident Summary

Concise description of alleged conduct, implicated parties, dates, and locations sufficient for an investigator to understand the core allegation without attachments.

Financial Information

Amounts, currencies, payment methods, recipient details, invoice numbers, and any bank or transaction references tied to the alleged payment.

Supporting Evidence

Field for attachment names and descriptions (emails, invoices, contracts, screenshots), and chain-of-custody notes for each exhibit.

Immediate Actions

Containment steps taken at discovery (suspension of payment, access restrictions, preservation hold) and the person responsible for each action.

Routing & Audit

Record of reviewers, dates reviewed, disposition codes, and an audit trail showing who viewed or edited the form.
