Common Preparation Mistakes to Avoid

  • Entering a mismatched legal name or incorrect TIN frequently triggers payer rejections, backup withholding, and manual remediation that delay payments and increase administrative cost.
  • Providing incomplete bank routing or account numbers results in failed ACH transactions, require rework, and can expose the organization to reconciliation overhead and customer dissatisfaction.
  • Omitting proof of signer authority—such as corporate resolutions or power of attorney—increases legal risk and may invalidate the document for vendor onboarding or contract acceptance.
  • Failing to retain an audit trail or signed copy complicates audits and dispute resolution; ensure timestamps, IP data, and signer attribution are preserved.
