What the Business CFI Form Is and When It’s Used

The Business CFI Form is a standardized company financial information form used to collect key financial, ownership, and operational data from a business during underwriting, vendor onboarding, or regulatory due diligence. It typically requests balance sheet figures, revenue and profit metrics, tax identification numbers, ownership percentages, authorized signatory details, and supporting schedules. Organizations use it to assess creditworthiness, verify compliance, and complete risk assessments. The form is often integrated into procurement, lending, or compliance workflows and can be completed on paper or electronically through an eSignature platform compliant with ESIGN and UETA.

Why a Standardized Business CFI Form Matters

The Business CFI Form centralizes financial facts needed for decisions such as credit approval, vendor acceptance, or regulatory reporting. Standardizing submission reduces review time, improves accuracy, and creates an auditable record that supports compliance with ESIGN, UETA, and industry-specific rules.

  • If the legal name and EIN do not match IRS records, payers may apply backup withholding and delay payments pending corrected documentation.
  • Using initials, unsigned fields, or having an unauthorized signer can void acceptance and require a re-execution step with notarization.
