Organizations use the document to create an auditable record for operational change, procurement, or service requests. The format supports electronic completion and secure eSignatures when parties consent under applicable U.S. electronic-signature laws.

Core Sections to Include for a Professional Record

Core sections of the Business Communication Polycom Document define parties, subject, detailed message, action items, attachments, and signature blocks to ensure completeness and traceability.

Parties

List full legal names, titles, departments, and contact details for sender and recipient. Include corporate entity names and the email or phone used for authorized correspondence to avoid ambiguity in recordkeeping.

Subject Line

Provide a concise subject that summarizes purpose and date. Use standardized prefixes (e.g., 'Service Notice', 'Change Request') to make classification and automated routing reliable across teams and systems.

Message Body

Action Items

Specify tasks, responsible parties, delivery dates, and priority level. Use MM/DD/YYYY format for dates. Include acceptance criteria and escalation contacts to reduce follow-up delays and clarify responsibility.

Attachments

List filenames, brief descriptions, and file formats. For technical logs or diagrams, note version numbers and timestamps so reviewers can match attachments to the message context and preserve traceability.

Signature Block

Include printed name, title, organization, and date. For institutional signers show delegated authority lines. Indicate whether signature may be provided electronically and which authentication method is acceptable.
