Consequences of Errors or Omitted Information

Incorrect Address: Delivery failure may void notice
Missing Signature: Document may lack enforceability
Wrong Date: Alters effective obligations and deadlines
No Notarization: May invalidate documents requiring acknowledgement
Privacy Violation: HIPAA or data breach risk if PHI exposed
Late Submission: Possible contractual penalty or waiver risk

Common Preparation Pitfalls to Avoid

  • Failing to reference the correct contract or invoice number leads to confusion and additional follow-up from recipients.
  • Using ambiguous language about required actions or deadlines increases the chance of noncompliance or dispute.
  • Relying on informal sign-offs or initials when the contract requires an authorized signature can render the communication ineffective.
  • Not retaining a complete copy with the audit trail makes it difficult to demonstrate proper service or intent later.
