What the Business Document SUR PQ Document Is

The Business Document SUR PQ Document is a supplier pre-qualification and procurement questionnaire used to collect standardized company data, compliance attestations, financial and insurance details, and references prior to awarding contracts. It centralizes supplier credentials and risk information so procurement teams can compare respondents consistently, verify minimum requirements, and document selection reasoning. The form typically includes company identity, tax identifiers, insurance certificates, safety records, ESG attestations, and a signature block for authorized representatives. It is intended to create an auditable record of supplier suitability for commercial engagements.

Why a Properly Completed SUR PQ Document Matters

Completing the SUR PQ Document accurately reduces procurement risk, speeds evaluation, and creates an auditable compliance record for contracting decisions under company policy and regulatory requirements.

Why a Properly Completed SUR PQ Document Matters

Who Typically Prepares and Reviews This Document

Procurement, vendor management, compliance, and contract teams commonly use the SUR PQ Document to screen potential suppliers before onboarding.

  • Procurement teams and buyers responsible for vendor selection and contract awards
  • Vendor management and compliance officers assessing insurance, tax, and regulatory status
  • Contract administrators and legal reviewers validating terms and signature authority

External suppliers complete the form; internal reviewers use the responses to approve, reject, or route suppliers for further due diligence.
