What the Business FAI Form Is and when it's used

The Business FAI Form is a standardized record used to document a first-article inspection, initial acceptance, or functional acceptance for products, services, or vendor deliverables. It captures identifying details, inspection findings, measurements against specified tolerances, nonconformances, and signatory confirmation that the item meets contract requirements or requires corrective action. Organizations use the form to create a verifiable acceptance trail between supplier and buyer, support warranty and quality control claims, and establish the baseline for future change control or corrective action workflows.

Why a clear Business FAI Form matters

A precise Business FAI Form reduces ambiguity about acceptance criteria, speeds resolution of defects, and creates an auditable record for contractual obligations. It helps allocate responsibility, supports invoice release decisions, and lowers the risk of downstream rework or warranty disputes.

Why a clear Business FAI Form matters

Who prepares and who signs the Business FAI Form

Typical users include quality engineers, receiving inspectors, vendor representatives, and contract managers responsible for acceptance decisions.

  • Quality engineer or inspector: completes measurements, records nonconformances, and signs to attest inspection results.
  • Vendor or supplier rep: verifies traceability data, confirms corrections when required, and signs acceptance or corrective action commitments.
  • Contract manager or buyer: reviews results, authorizes acceptance or escalation, and approves payment or holds.

The form must be signed by authorized parties; see the 'Who Has Authority to Sign' and 'Notarization and Witness Requirements' sections for role-specific guidance.

Essential parts of a professional Business FAI Form

A professional form structures identification, measurement data, disposition decisions, and signatures so every decision and correction is traceable.

Header

Supplier name, purchase order number, part number, serial or lot numbers, and inspection date so records are unambiguous and searchable.

Acceptance Criteria

Reference to drawings, specifications, and tolerance limits with clause citations so measured values can be evaluated against contract requirements.

Measurement Log

Tabular fields for dimension, measured value, measurement method, and inspector initials to maintain traceable evidence of inspection.

Nonconformance Record

Clear description of defects, severity, disposition (rework/repair/reject), and required corrective actions with responsible party and due date.

Disposition

Pass/fail or conditional acceptance checkbox and required references to QA hold tags, quarantine locations, or shipping instructions.

Signature Block

Designated signer names, titles, signature lines and signature dates to confirm review and acceptance or to document escalation steps.

Step-by-step: filling and approving a Business FAI Form

Follow a consistent sequence to ensure inspection results are complete and legally defensible.

  • 01
Prepare document: Upload PO and drawings before inspection.
  • 02
Conduct inspection: Measure, photograph, and record findings.
  • 03
Record disposition: Select pass, conditional, or fail.
  • 04
Sign and route: Collect authorized signatures and notify stakeholders.

How to configure an online FAI workflow

Set fields, routing, and authentication so the form matches contractual requirements and internal controls.

Field Configuration
Part Number Required, single-line text
Measurement Table Repeatable table with units
Disposition Radio buttons: Pass / Conditional / Fail
Signers Sequential routing with signer roles

Typical routing for electronic FAI submission

A clear routing order reduces signer friction and preserves an auditable trail.

  • Originator: Prepares form and attaches specs.
  • Inspector: Completes measurements and evidence.
  • Supplier: Reviews and confirms corrective actions.
  • Contract Manager: Approves disposition and closes file.

Technical considerations for eSubmission and signatures

Choose a platform that supports secure authentication, a tamper-evident audit trail, and exportable signed PDFs.

  • Authentication: Email, SMS code, or stronger
  • Audit Trail: IP, timestamp, and actions
  • File formats: PDF, DOCX supported

Ensure the chosen provider can meet contract or regulatory requirements such as HIPAA, 21 CFR Part 11, or customer-specific strong authentication.

Typical timelines and processing expectations

Set clear deadlines to avoid payment delays and hold costs.

Inspection window:

Complete within 7 calendar days of delivery.

Correction period:

Supplier to respond within 14 days of notice.

Reinspection:

Schedule within 7 days after correction.

Approval to invoice:

Invoice release on formal acceptance date.

Retention of evidence:

Keep inspection records until warranty expires.

Common preparation mistakes to avoid

  • Missing or partial serial numbers that break product traceability and delay acceptance.
  • Recorded values without units or precision that prevent objective compliance checks.
  • Absent or unsigned corrective-action commitments that leave disputes unresolved.
  • Uploading incorrect drawings or old revision numbers that invalidate the inspection.

Consequences of incorrect or incomplete FAI submissions

Payment delay: Hold or reject invoices
Warranty risk: Void or limit coverage
Contract breach: Damages or termination
Reinspection cost: Supplier expense
Regulatory exposure: Fines or remediation
Reputational harm: Client relationship loss

Essential data points to collect on the form

Entity: Legal business name
Identifiers: PO, part, and serial numbers
Inspector: Name and certification
Results: Measured values and pass/fail
Evidence: Photos or attachments
Signatures: Signer name and date
