What the Business Form A PMK Is and When It’s Used

Business Form A PMK is a standardized business authorization and onboarding form commonly used to record vendor details, payment instructions, and authorized signatory information for commercial relationships. Organizations use it to collect legally required identification data, tax information, and authorization to initiate payments or integrate a vendor into procurement and accounts payable systems. The form is designed to be machine-readable and compatible with electronic workflows, enabling secure collection, verification, and retention of information necessary to start commercial transactions and comply with recordkeeping obligations.

Why using a clear Business Form A PMK matters

A complete Business Form A PMK reduces onboarding delays, lowers payment errors, and creates a consistent record for audit and compliance purposes. It centralizes key business, tax, and authorization data so teams can verify identity, validate taxpayer identification numbers, and document the authority to accept or disburse funds.

Why using a clear Business Form A PMK matters

Who typically completes and relies on this form

The Business Form A PMK is used by internal teams and external partners involved with vendor onboarding and payments.

  • Procurement and accounts payable teams — collect vendor tax IDs, bank details, and contact information to set up suppliers in ERP systems.
  • Vendors and contractors — provide legal name, W-9 or W-8 status, payment instructions, and authorized signatory details.
  • Compliance and finance reviewers — verify TINs, backup withholding triggers, and evidence of authorization for payments.

Accurate completion by both submitting parties and reviewers prevents payment delays, backup withholding risk, and downstream reconciliation issues.

I-9 Noncompliance: If onboarding relates to employment, I-9 errors can result in civil penalties under 8 CFR §274a.2

Common mistakes to avoid when preparing the form

  • Using a colloquial or shortened business name that does not match IRS records leading to failed TIN matches.
  • Failing to attach a required W-9/W-8 before enabling payments, causing reporting gaps and delays.
