Entity identifiers

Include full registered name, DBA if any, EIN/Tax ID, and state or country of formation to enable tax and vendor checks.

Contact data

Collect primary contact name, role, phone, and email. Clearly identify who will accept notices and manage follow-up.

Purpose and scope

State the reason for the submission (vendor onboarding, contract execution, registration) and limit the form to that scope to avoid ambiguity.

Authority statement

Provide a signer capacity clause confirming the signer's authority to bind the organization, plus any required corporate resolution reference.

Attachments and exhibits

List required supporting documents (formation, EIN letter, W-9) and specify acceptable file types and minimum resolution for scans.

Audit and versioning

Capture timestamps, signer IP, and version history. Maintain a single signed PDF with an appended completion certificate for verification.
