Regulatory Fine: Noncompliance can lead to administrative penalties

Common preparation errors to avoid

  • Using an informal or vague purpose statement that requires multiple follow-ups and slows resolution.
  • Providing mismatched names or TINs that force re-verification and may trigger backup withholding or payment delays.
  • Omitting attachments referenced in the letter, causing recipients to treat the request as incomplete.
  • Failing to have an authorized signer or notarization where required, creating enforceability concerns.
