Essential elements of a professional Business Information Cobra Info

A complete form combines administrative identifiers, event data, contact details, and signature fields so notices can be produced and actions tracked consistently.

Employer Identifier

Employer legal name and EIN must be present and match payroll and tax records to avoid misrouting and to support accurate tax reporting and plan identification.

Plan Identification

Clearly state the plan name, plan number (if used), and plan administrator contact to ensure notices reference the correct benefit and allow beneficiaries to reach plan contacts.

Qualifying Event Details

Describe the qualifying event type, the event date in MM/DD/YYYY format, and any supporting notes to establish the COBRA eligibility start point and coverage window.

Coverage Dates

Specify the coverage loss date and proposed COBRA start and end dates so premiums and election timelines can be calculated without ambiguity.

Beneficiary Contact

Collect the beneficiary's full name, mailing address, email, and telephone to support delivery of election notices and any required follow-up communications.

Authorization and Signature

Include an authorized employer signature block with printed name, title, date, and a signer email; this verifies the employer's authorization to notify and bill under the plan.

Security and compliance data points to include

Encryption: AES-256 at rest
Transport: TLS 1.2/1.3 in transit
Access Controls: Role-based permissions
Audit Trail: Immutable action log
HIPAA: BAA available if needed
Authentication: Support for MFA

Common preparation pitfalls to avoid

  • Using inconsistent plan names across records, which leads to misapplied notices and enrollment errors during administration and audit review.
  • Entering incorrect event or effective dates, creating confusion about election windows and potentially exposing the employer to compliance risk or contested coverage periods.
  • Providing incomplete beneficiary contact information, which delays notice delivery and may cause a loss of coverage rights if election materials are not received.
  • Failing to secure signed authorization or relying solely on initials, which can undermine the legal enforceability of the employer-provided attestations.

Potential penalties and liability concerns

Late Notice Penalty: Administrative fines possible
Coverage Disputes: Benefit claims denial risk
Recordkeeping Failure: Compliance citations likely
Incorrect Billing: Repayment or audit exposure
Privacy Breach: HIPAA enforcement risk
Legal Costs: Attorney fees and defense
