What the Business Information Report MIR Is

The Business Information Report MIR is a standardized corporate data summary used to collect and verify essential company details for regulatory, contracting, or vendor onboarding purposes. It typically gathers legal entity name, formation jurisdiction, tax identification numbers, principal office address, ownership and management contacts, and relevant licenses or certifications. Organizations use the MIR to centralize verification, reduce repetitive requests, and establish a consistent record for audits or compliance reviews. The form is not a tax return but feeds into administrative, legal, and procurement workflows that rely on accurate business identity data.

Why a Clear Business Information Report MIR Matters

A complete MIR reduces onboarding friction, supports compliance checks, and creates a single source of truth for corporate identity and tax reporting needs.

Why a Clear Business Information Report MIR Matters

Who Typically Completes or Requests the MIR

The MIR is commonly completed by internal business owners or external vendors during onboarding, contracting, or compliance reviews.

  • Business owners and authorized officers: Provide legal entity details and signatory authority for contracts and banking.
  • Accounting and tax teams: Verify EIN, tax classification, and reporting details to prevent backup withholding.
  • Procurement and vendor managers: Use MIR data to set up vendors, validate certificates, and perform risk screening.

Keeping the MIR accurate and current supports tax withholding, payment setup, and regulatory due diligence across departments.

Representative People Who Fill or Approve the MIR

Chief Financial Officer

The CFO or finance director verifies tax identification, corporate structure, and accounting contact information; they ensure the MIR aligns with filings and withholding requirements and may delegate completion to accounting staff.

Authorized Company Officer

An officer with signing authority (president, CEO, managing member) confirms legal entity details, signs attestations, and authorizes third-party disclosures or vendor onboarding per corporate policy.

Core Sections Found in a Professional MIR

A complete MIR organizes company identity, ownership, tax data, contact points, regulatory credentials, and attestation/signature sections so receiving parties can quickly verify authority and compliance status.

Entity Identification

Legal name, DBA, formation state, and business structure (e.g., LLC, C-Corp) to match public filings and registration records.

Tax Details

Employer Identification Number (EIN), tax classification (S-Corp, partnership), and backup withholding indicators for payer systems.

Principal Address

Registered office and mailing address, including street, city, state, and ZIP; P.O. boxes alone are insufficient for some verifications.

Ownership and Control

List of owners or officers with ownership percentages and roles to satisfy KYC or beneficial ownership screening.

Licenses and Credentials

Professional licenses, state registrations, or industry certifications that affect eligibility to contract or receive payments.

Attestation and Signature

Signed declaration of truth, effective date, and signer authority with space for notarization or witness if required.

  • Failing to attach required licenses or resolutions leads to repeated requests.
  • Not verifying signer authority increases risk of contract disputes or rescission.
