What the Business LNP Form Is and when it’s used

The Business LNP Form (Local Number Portability authorization for businesses) is a carrier-level authorization used to move telephone numbers from one service provider to another. It documents the account owner, account number, numbers to be ported, current carrier, billing and regulatory identifiers, and an authorized signature or letter of authorization. Carriers use the form to verify ownership and obtain consent before initiating a port. The form may also require identity verification, a copy of a recent bill, and explicit authorization to avoid service disruption during transfer.

Why a correct Business LNP Form matters for your business

Using a correct Business LNP Form reduces downtime, preserves existing phone numbers, and documents consent required by carriers. It centralizes account and regulatory details so porting proceeds on schedule and minimizes the risk of rejected requests or billing disputes during carrier transition.

Why a correct Business LNP Form matters for your business

Who typically completes the Business LNP Form

Typical users include IT managers, telecom administrators, and authorized business representatives completing number-transfer authorizations with a carrier.

  • IT and telecom teams managing site migrations and unified communications transitions.
  • Finance or billing departments coordinating account numbers, invoices, and billing transfer approvals.
  • Service providers' porting agents or vendors preparing documentation and verifying authorization.

Confirming the correct signer and up-to-date account details prevents delays and reduces the need for follow-up documentation or carrier validations.

Core elements required on a Business LNP Form

Important elements in a Business LNP Form ensure legal authorization, accurate number lists, and traceable signatures to satisfy carrier verification and prevent service interruptions.

Port Authorization

Explicit written consent that authorizes the gaining carrier to request transfer of specified telephone numbers. It should include account references and a clear statement assigning porting rights to the gaining provider.

Number Inventory

A complete list of numbers to be ported, presented one per line or as a CSV attachment. Include country and area codes, extensions where applicable, and any routing identifiers.

Account Details

The donor account number, billing name and address, and any customer or service identifiers carriers use to verify ownership. Consistency with the carrier bill is critical to avoid denials.

Identity Proof

A copy of a recent bill, government ID, or other documentation proving the account holder’s identity. Carriers may require notarization or additional verification for corporate accounts.

Signatory Authority

A statement identifying the individual signing the form and their authority (title or role). For third-party agents, include a power of attorney or explicit authorization from the account owner.

Audit Trail

A dated signature, IP address or notarization record, and a record of communications. Retain the audit trail to support disputes and regulatory compliance during and after the port.

Step-by-step process to prepare and submit the Business LNP Form

Follow these steps to prepare and submit a Business LNP Form for carrier number porting.

  • 01
Gather Info: Collect account numbers, recent bill copy, and the full list of numbers.
  • 02
Complete Form: Fill every required field and attach number inventory or CSV.
  • 03
Authenticate: Provide signer ID or notarization as required by carrier.
  • 04
Submit: Send to gaining carrier or authorized porting contact with supporting documentation.

Configuring a digital workflow for Business LNP Form submission

Configure digital workflows to collect required data, capture signatures, and attach number inventories or bills.

Workflow Setting / Field Name Configuration
Authentication Method Email link or SMS code
File Format PDF for forms; CSV for number inventory
Signature Capture E-sign with audit trail; RON if notarization needed
Notifications Email confirmations to admin and signer

How the porting workflow moves between carriers

An LNP port follows a standard flow where the gaining carrier requests the number and the donor carrier validates ownership before activation.

  • Request: Gaining carrier submits port order using the provided Business LNP Form.
  • Validation: Donor carrier checks account and authorization against billing and records.
  • Scheduling: A port date is confirmed; time windows vary by carrier.
  • Completion: Numbers are transferred and services routed to the new provider.

Technical platform requirements for eSubmission and eSign

Use platforms that support PDF, DOCX, and CSV imports, secure eSign workflows, and integrations with CRMs to streamline porting.

  • File Formats: PDF, DOCX, CSV supported
  • Integrations: Salesforce, NetSuite, Microsoft 365
  • Security: AES-256 at rest; TLS 1.2/1.3

How to download, save, and package completed LNP forms

Save and distribute completed LNP forms in standard formats to preserve records, facilitate carrier intake, and meet retention requirements efficiently.

PDF/A Export

Export a PDF/A version to create an archival, tamper-evident copy. Include embedded audit trail or attach the certificate of completion to demonstrate signature attribution and timestamps.

DOCX Backup

Keep a DOCX editable copy for internal recordkeeping and future edits. Mark it 'Do Not Submit' when retaining an editable draft to avoid accidental carrier submission.

CSV Number List

Provide a CSV containing columns for telephone number, account number, port type, and any carrier identifiers. CSVs simplify bulk imports for carriers and reduce manual error.

Audit Record

Save the signature certificate, IP logs, and email confirmations as a single bundle. These documents support dispute resolution and regulatory audits if porting issues arise.

Typical timing and scheduling notes for port requests

Timing varies by carrier; submit completed LNP forms early and allow for validation, scheduling, and possible follow-up.

Form Preparation:

Complete and verify fields before submission to avoid rejections.

Submission Window:

Send at least three business days before desired port date.

Donor Validation Period:

Carrier verification may take one to five business days depending on account complexity.

Scheduled Port Date:

Carrier confirms a firm port date; avoid making number changes beforehand.

Post-Port Checks:

Verify service routing and billing within 24 to 72 hours after completion.

Common preparation mistakes to avoid

  • Incomplete account numbers or mismatched billing names leading to donor carrier rejections and delayed ports.
  • Incorrect formatting of phone numbers (missing country or area codes) causes import failures in bulk processing.
  • Missing authorization from the account holder or inadequate proof of authority triggers additional verification steps.
  • Submitting multiple versions or unsigned drafts can create record confusion and increase dispute risk with carriers.

Risks and negative outcomes from incorrect forms

Service Interruption: Temporary loss of phone service
Port Rejection: Request denied; repeat process required
Billing Errors: Incorrect billing during transition
Compliance Exposure: Regulatory fines or audits possible
Customer Impact: Missed customer calls and reputational harm
Extended Delays: Extended troubleshooting and legal costs
