What the Business MACD Template Is

The Business MACD Template is a standardized internal request form used to Move, Add, Change, or Delete business assets, services, user accounts, or physical locations. It collects requester details, description of the requested change, effective date, impacted systems or locations, approval signatures, and implementation steps. Organizations use the template to document authorization, provide a consistent intake record, and create a clear audit trail for operations teams, vendors, and compliance reviews.

Why a Formal MACD Template Matters

A structured Business MACD Template reduces errors, documents approvals, and establishes a repeatable operational workflow. It helps limit unauthorized changes, speeds vendor coordination, and preserves an audit trail for IT, facilities, finance, and compliance teams while clarifying responsibilities across departments.

Why a Formal MACD Template Matters

Teams That Commonly Complete a MACD Request

Typical requesters and approvers span functions that manage assets, accounts, services, or facilities.

  • IT teams requesting account or access changes for users and systems.
  • Facilities or operations teams coordinating moves, desk changes, or site closures.
  • Finance and procurement for additions, cancellations, or vendor billing changes.

Larger organizations route MACD requests through ticketing systems and retain signed records for operational audits and vendor reconciliation.

Core Sections to Include in a Professional MACD Template

Design the template so each change request captures authorization, scope, schedule, impacted assets, recovery steps, and verification. Clear sections reduce back-and-forth and shorten implementation timelines.

Request Summary

Concise description of the requested Move/Add/Change/Delete and the business reason for the change.

Requester Details

Name, department, contact phone/email, and business unit to ensure clear ownership for follow-up.

Impact Scope

List affected systems, users, locations, asset tags, or account IDs to prevent unintended disruptions.

Schedule & Dates

Requested effective date, preferred time window, and blackout periods for scheduling.

Approvals

Signature blocks or eSignature fields for required approvers plus escalation path if denied.

Implementation Notes

Step list, rollback plan, validation steps, and verification checkpoints after completion.

Step-by-Step: Submitting a Business MACD Request

Follow these steps to complete and route the MACD request for timely implementation and auditability.

  • 01
Prepare Request: Complete all required fields and attach supporting documents.
  • 02
Obtain Approvals: Get required signatories to sign or eSign before submission.
  • 03
Submit to Workflow: Upload the form to the ticketing system or vendor portal.
  • 04
Confirm Completion: Verify changes and record completion details in the form.

Common Digital Workflow Settings for Online Completion

Configure the online workflow so fields, approvals, and notifications align with internal SLAs.

Field Configuration
Approval Flow Sequential approval with conditional escalation on rejection
Notifications Automatic email and in-app alerts to approvers and implementers
Attachments Require supporting files before submission
Audit Trail Enable full event logging and timestamp capture

Where to Send Completed MACD Requests

Determine the destination based on the change type and the owning function to ensure correct routing.

  • IT Ticketing: Submit to the IT service desk for account and system changes.
  • Facilities Portal: Send physical move or space changes to facilities management.
  • Vendor Portal: Upload vendor-managed changes to the vendor's service portal.
  • Finance Records: Forward billing or procurement changes to accounts payable when required.

Digital Signing and eSubmission Considerations

Choose an eSignature platform that supports the required authentication, audit trail, and integrations for your MACD workflow.

  • Integrations: Salesforce, NetSuite, Google Workspace, Microsoft 365 supported
  • File Formats: Accepts PDF, DOCX, HTML, and Excel formats
  • Authentication: Options for email OTP, SMS, or advanced signer verification

Ensure the chosen platform captures timestamps, signer attribution, and a tamper-evident PDF for retention and audits.

Typical Timelines and Processing Expectations

Different change types carry different SLAs; set expectations for submitters and approvers to avoid operational delays.

Request Submission Window:

Submit non-emergency requests at least 3 business days before the effective date.

Internal Approval SLA:

Approvers should respond within 2 business days to avoid workflow escalation.

Vendor Implementation Time:

Third-party vendors may require up to 10 business days for completion.

Emergency Changes:

Document emergency approvals immediately and follow with a formal MACD record.

Confirmation and Closeout:

Implementers must update the request and confirm completion within 1 business day.

Key Milestones from Request to Completion

Track milestones so stakeholders can monitor progress and identify bottlenecks during the MACD lifecycle.

02

Essential Data Elements to Capture

Requester: Full name and contact
Department: Business unit or cost center
Change Type: Move/Add/Change/Delete
Asset ID: Serial or account identifier
Effective Date: MM/DD/YYYY format
Authorizer: Approver name and signature

Risks and Consequences of an Incorrect MACD Request

Service Disruption: Unscheduled downtime or loss of access
Financial Loss: Incorrect billing or duplicate charges
Compliance Violation: Regulatory exposure for protected data mishandling
Data Breach: Unauthorized access due to improper deprovisioning
Contract Breach: Vendor SLA or contractual penalties
Delayed Projects: Missed deadlines and downstream impacts

Common Mistakes to Avoid When Preparing a MACD

  • Leaving asset or account identifiers blank, which causes incorrect target changes and implementation delays.
  • Not securing required approvals before submission, creating audit exceptions and rollback work.
  • Using ambiguous dates or time windows that conflict with maintenance windows and business operations.
  • Failing to attach vendor quotes or tickets, which prompts repeated information requests and extended SLAs.

Real-World Examples of MACD Templates in Use

Organizations use structured MACD forms to speed execution and maintain compliance across distributed teams.

Optica Ventures (IT)

Optica standardized MACD intake to eliminate informal requests and missing data.

  • The interface is simple and easy-to-use for the team.
  • As a result, the company reduced back-and-forth with vendors and shortened implementation cycles while preserving a full audit trail for operational reviews.

Martin Properties (Facilities)

A property management firm consolidated move and access changes into one template.

  • It allowed remote approvals and verification.
  • They processed onsite moves and tenant account changes online, maintaining compliance and reducing site visit requirements for routine assignments.

Who Typically Signs or Approves a MACD

IT Manager

The IT Manager reviews technical impact, confirms asset identifiers, and signs to approve changes that affect systems or access. They coordinate implementation windows and validate post-change operations to minimize downtime and security risks.

Facilities Director

The Facilities Director approves moves, space changes, and physical asset relocations. They verify site readiness, schedule movers, and confirm safety checks to ensure operational continuity.
