What the Business Name Request Form Is and When it’s Used

The Business Name Request Form is a formal submission used to register, reserve, or declare an assumed business name (DBA/trade name) with a state or local filing office. It captures the legal entity name, the requested trade name, entity type, formation jurisdiction, owner or authorized signer, and contact details. Typical filers include sole proprietors, partnerships, LLCs, and corporations. Agencies receiving the form may be a Secretary of State, county clerk, or municipality; acceptance establishes a public record and helps prevent conflicting registrations.

Why a Proper Business Name Request Form Matters

A correctly completed form prevents registration delays, reduces the risk of name conflicts, creates a public record for banking and tax purposes, and documents authorized signers. Clear records also support compliance with state filing rules and reduce downstream administrative and tax complications.
