What the Business Performance MOC Report Is

The Business Performance MOC Report (Management of Change Report) documents planned or completed organizational changes that affect business performance, operations, or controls. It records the rationale, scope, impacted processes, metrics to monitor, risk assessments, approvals, and implementation timelines so stakeholders can evaluate effects on productivity and compliance. Typical uses include process redesign, system deployments, role or responsibility shifts, KPI baseline updates, and control changes. The report ties expected outcomes to measurable indicators and creates an audit trail for later review and continuous improvement.

Why a Structured MOC Report Matters

A clear MOC Report aligns stakeholders, reduces operational disruption, and preserves evidence of decision-making. It helps quantify performance impact, supports risk mitigation, and documents approvals for audits or regulatory review. When executed correctly it provides a single source of truth for change scope, owners, timelines, and measurable outcomes.

Why a Structured MOC Report Matters

Who Typically Completes and Reviews an MOC Report

Start the report with stakeholders who own the change and governance reviewers identified clearly, then circulate for technical, legal, and finance input.

  • Change Owner (Business Lead) — accountable for scope, objectives, resource estimates, and post-change validation.
  • Operations / Process SME — documents process impacts, required training, and operational risk controls.
  • Legal / Compliance Reviewer — verifies regulatory risk, contract impacts, and required disclosures.

Finalize signatory roles and approval sequence before implementation so authorization and escalation paths are unambiguous.

Core Sections to Include in a Professional MOC Report

A complete Business Performance MOC Report has discrete sections to capture decision rationale, risk controls, metrics, and approvals. Use a structured template so reviewers can find evidence quickly and compare results across changes.

Summary

One-paragraph overview of change, objective, and expected business impact, including scope and timeline.

Scope & Actors

List affected departments, systems, external vendors, and the change owner with contact information.

Risk Assessment

Identify operational, financial, legal, and compliance risks with mitigation steps and owners.

Performance Metrics

Baseline KPIs, targets, data sources, and monitoring frequency to measure success or regression.

Approval Log

Role-based approvals, dates, and conditional sign-offs required before and after implementation.

Post-Implementation Review

Validation checklist, results, lessons learned, and recommended next steps for continuous improvement.

Step-by-Step: Completing the MOC Report

Follow a linear workflow from scoping to post-implementation review to maintain traceability and accountability.

  • 01
Draft: Document scope, objectives, and baseline metrics.
  • 02
Assess: Run risk assessment and mitigation planning.
  • 03
Approve: Collect required role-based approvals before execution.
  • 04
Review: Conduct post-implementation validation and record outcomes.

Configuring an Online MOC Workflow

Set up the digital workflow to mirror your approval sequence and control validation steps before launching the change.

Field Configuration
Template Creation Create reusable template with required fields and sections.
Conditional Fields Show or hide fields based on change type to reduce signer friction.
Authentication Use email, SMS code, or advanced signer authentication as required.
Integrations Connect to CRM or ticketing systems for automatic data population.

Where to Submit or File the Completed Report

Route the signed report to operational owners, compliance, and records management according to your internal retention policy.

  • Operational Archive: Store final report in your document repository or change management system.
  • Compliance Folder: Send a copy to legal or compliance for review and audit readiness.
  • Finance Records: Provide finance with metrics and cost changes for accounting updates.
  • Executive Reporting: Include summary in executive dashboards or monthly performance packs.

Distribution and Digital Signing Considerations

Decide how stakeholders will receive and sign the report and which authentication level is required before distributing the document.

  • File Formats: PDF, DOCX, XLSX supported
  • Integrations: Salesforce, NetSuite, Google Workspace
  • Auth Options: Email link, SMS code, KBA

Use a platform that preserves an audit trail, supports required authentication, and produces a tamper-evident signed copy for records.

Typical Timelines and Deadlines for an MOC Report

Timelines vary by organization and risk profile. Establish target deadlines for each phase to keep implementation on schedule and to meet audit expectations.

Common Preparation Errors to Avoid

  • Incomplete scope definitions that omit affected systems or teams, causing unplanned downstream work and missed risks.
  • Missing or inconsistent baseline metrics that prevent valid before/after comparisons and obscure whether objectives were achieved.
  • Collecting signatures without proper identity evidence or timestamps, weakening the document's auditability during regulatory or internal review.
  • Delaying post-implementation review past the monitoring window, which allows regressions to persist and reduces learning value.

Consequences of Inaccurate or Incomplete Reports

Operational Loss: Increased downtime and unplanned costs
Regulatory Risk: Compliance findings and remediation costs
Audit Weakness: Failed internal or external audits
Contract Breach: Vendor or customer liability exposure
Reputational Harm: Stakeholder trust erosion
Financial Impact: Missed KPIs and budget variances
