What the Business POI Template Is and when it’s used

The Business POI Template is a standardized proof-of-identity form used to verify a legal entity or authorized representative during business onboarding, vendor setup, banking, or contract execution. It collects the minimum identity elements required by many organizations: entity name, employer identification number (EIN) or TIN, representative name, government ID details, and supporting corporate documents to confirm authority to act.

Why a clear Business POI Template matters

A concise POI template reduces onboarding delays, lowers manual verification errors, and creates an auditable record of identity checks for compliance and risk teams while documenting who authorized the transaction.

Why a clear Business POI Template matters

Typical users and roles that complete this template

The Business POI Template is most often completed by corporate representatives, procurement teams, and compliance staff during onboarding or transaction setup.

  • Accounts payable and procurement staff completing vendor verification during onboarding and payment setup.
  • Authorized officers and managers providing identity details to open bank accounts or accept contracts.
  • Compliance or legal teams documenting identity verification for AML, KYC, or contractual authority.

Use this document to create a consistent audit trail showing who provided identity information and what supporting records were verified.

Stepwise completion process for the Business POI Template

Follow these four steps in order to collect, verify, and record identity information consistently.

  • 01
Collect: Request entity and representative ID and supporting documents.
  • 02
Verify: Compare IDs to corporate records and perform KYC checks if required.
  • 03
Record: Save verified documents and note verification method and date.
  • 04
Sign: Obtain dated signature from the authorized representative.

Configuring an online workflow for Business POI collection

Set up your digital workflow to automate requests, attach required documents, and capture audit data for each submission.

Field Configuration
Entity Name Field Required text field; auto-validate against entity list
EIN/TIN Field Masked numeric field; format validation
Document Upload Require PDF or image; allow multiple attachments
Signer Authentication Enable email or SMS code authentication

Technical considerations for digital POI collection

Choose a platform that supports secure uploads, optional advanced authentication, and an immutable audit trail.

  • File formats: PDF, DOCX, JPG support
  • Authentication: Email, SMS, or KBA
  • Integrations: CRM and cloud storage

Confirm your provider supports encryption in transit and at rest, audit logs, and any industry-specific compliance (for example HIPAA for healthcare) before collecting sensitive POI data.

Where to send and how verified POI is routed

A typical routing sequence moves the completed POI from submitter to verifier to records storage with automated notifications at each step.

  • Submitter: Uploads POI and supporting files via secure link
  • Verifier: Performs identity checks and marks status
  • Approver: Legal or compliance confirms authority if required
  • Archive: Signed POI stored in secure records repository

Essential elements every Business POI Template should include

Design the template to capture the minimum verifiable facts, make verification repeatable, and produce a tamper-evident record of the check.

Entity Identifier

Legal entity name and EIN/TIN fields that match formation documents to reduce ambiguous matches during vendor onboarding and banking checks.

Representative Details

Full name, title, contact, and government ID information for the person signing or authorizing transactions on behalf of the entity.

Authority Evidence

Placeholders for corporate resolutions, operating agreements, or board minutes to demonstrate the signer’s authority to act for the business.

Document Uploads

Required PDF or image uploads for IDs and corporate records with file-type validation and size limits for reliable processing.

Verification Log

Fields to record verifier name, method used (manual, RON, third-party), date, and outcome to support audits and dispute resolution.

Signature and Date

Clear signature block with printed name, capacity, and MM/DD/YYYY date to establish intent and timing of authorization.

Security and compliance facts to capture with each submission

Encryption: TLS 1.2/1.3; AES-256
Audit Trail: IP, timestamp, action log
Access Controls: Role-based permissions
BAA Support: HIPAA BAA available
Retention Flag: Retention metadata
Accessibility: WCAG 2.0 AA

Common mistakes to avoid when preparing Business POI

  • Collecting inconsistent names between corporate records and government ID, which causes verification failures and delays.
  • Accepting expired identification documents that do not meet verifier or banking requirements, leading to rejected onboarding.
  • Failing to document signer capacity or missing a corporate resolution, which undermines authority to sign.
  • Storing POI files without encryption or proper access controls, raising privacy and compliance risks.

Risks and potential penalties from incorrect or incomplete POI

Operational Delay: Transaction hold
Financial Risk: Payment blockage
Regulatory Risk: AML/KYC breach
Contract Risk: Enforceability questions
Fraud Exposure: Impersonation losses
Reputational Harm: Loss of trust

Timing expectations for collecting and processing Business POI

Establish clear service-level timelines to minimize onboarding friction and to meet regulatory verification windows.

Request at Onboarding:

Ask for POI immediately when initiating a supplier or account relationship.

Present Within Days:

Require representative to present ID within three business days of request when practical.

Verification Window:

Complete identity checks and record results within five business days for typical cases.

Retention Start Date:

Retention begins on the verification date recorded on the form.

Periodic Recheck:

Revalidate high-risk relationships annually or when significant corporate changes occur.
