What a Business Problem Resolution Document Is

A Business Problem Resolution is a formal internal record that captures a reported operational issue, the assessed impact, root cause analysis, corrective actions, and sign-offs from responsible parties. It creates a single authoritative record that supports accountability, auditability, and downstream process improvements. Typically used for process failures, customer-impacting incidents, or compliance events, the document also establishes timelines for investigation, remediation, and verification of effectiveness. When completed, it should provide a clear chronology, evidence of actions taken, assigned owners, and an accessible audit trail for internal governance or external review.

Why documenting resolution matters

Maintaining a completed Business Problem Resolution reduces repeated failures, creates traceable evidence for audits or compliance reviews, and supports consistent change control. It helps organizations demonstrate remediation timelines and assignment of responsibility without relying on ad hoc emails or verbal updates.

Why documenting resolution matters

Typical owners and contributors

Teams collaborate on Business Problem Resolutions, with different roles contributing information, approvals, and verification at defined stages.

  • Operations teams gather incident details, impact metrics, and remediation steps for process-related issues.
  • Risk, compliance, or legal teams review regulatory exposure and ensure corrective actions meet policy requirements.
  • IT and engineering document technical root causes, deploy fixes, and confirm remediation through testing.

Clear role separation helps ensure timely investigation, documented approvals, and an auditable completion record for internal and external stakeholders.

Who can sign and approve

Operations Manager

An Operations Manager typically compiles incident facts, approves the remediation plan, and confirms operational closure. They validate impact metrics and accept responsibility for implementing process changes where required.

Risk Officer

A Risk or Compliance Officer assesses regulatory exposure, approves any required notifications, and signs off on corrective controls. Their approval indicates the organization has addressed compliance gaps to the satisfaction of policy and legal requirements.

Core sections every resolution should include

A professional Business Problem Resolution follows a consistent structure so reviewers can quickly find facts, decisions, and evidence. The following components form the minimum viable record for most organizations.

Issue Summary

Concise statement of the problem, date/time reported, and immediate symptoms. Use objective language and attach initial evidence such as logs, screenshots, or customer complaints.

Impact Assessment

Quantify affected customers, financial exposure, regulatory implications, and operational scope. Include measurable KPIs, estimated monetary impact, and business processes affected.

Root Cause Analysis

Document analysis method (e.g., 5 Whys, RCA), identified root cause(s), and supporting data. Avoid attribution without evidence and reference technical artifacts when applicable.

Remediation Plan

List corrective actions, owners, deadlines, and verification steps. Distinguish short-term containment from long-term fixes and include acceptance criteria for closure.

Approvals & Sign-off

Record name, title, signature, and date for each approver. For legally or regulatorily sensitive incidents, include signatures from counsel or designated compliance officers.

Audit Trail & Evidence

Attach documents, time-stamped logs, communications, and validation test results. The audit trail should show who made edits and when, and preserve original versions where required.

Step-by-step: completing a Business Problem Resolution

Follow these sequential steps to create a complete, auditable resolution record from report through closure.

  • 01
    1. Record Issue: Capture reporter, date, and a concise problem statement.
  • 02
    2. Assess Impact: Measure affected scope, customers, and regulatory risk.
  • 03
    3. Define Remediation: Assign actions, owners, and deadlines for fixes.
  • 04
    4. Verify & Close: Validate fixes, obtain approvals, and archive evidence.

Configuring an online workflow for this form

Map fields and approvals to an automated workflow so routing, reminders, and sign-offs occur without manual tracking.

Field Configuration
Initial Reporter Required; email-based submitter field
Auto-assign Owner Conditional routing by department value
Approval Sequence Sequential: Owner → Risk → Legal

Where to send and how records are routed

A clear routing path ensures responsibilities are visible and the final record is accessible to auditors and stakeholders.

  • Submit to Operations: Primary intake for incident details and attachments.
  • Route to Risk: Risk reviews exposure and recommends notifications.
  • Legal Review: Legal assesses regulatory or contractual implications.
  • Archive Location: Store final record in controlled repository.

Digital delivery and platform considerations

Choose systems that preserve audit trails, support required file types, and integrate with your document repository.

  • File formats: PDF, DOCX supported
  • Integrations: Salesforce, NetSuite, Google Workspace
  • Authentication: Email, SMS code, or SSO

Confirm the chosen platform supports exportable audit reports, retention policies, and any required regulatory controls before deployment.

Common mistakes to avoid when preparing the form

  • Incomplete evidence: attaching a summary without logs or screenshots delays technical validation and may force re-opened investigations.
  • Unclear ownership: assigning vague responsibilities slows remediation and complicates tracking when multiple teams think someone else owns the task.
  • Vague remediation: describing fixes as 'will be addressed' without deadlines or acceptance criteria prevents objective verification and closure.
  • Missing approvals: failing to obtain required compliance or legal sign-off can expose the organization to regulatory risk during audits.

Risks and consequences of incorrect or missing records

Operational repeat: Recurring incidents
Regulatory exposure: Fines or notices
Contract breaches: Customer liability risk
Audit findings: Negative compliance reports
Evidence gaps: Disputed remediation
Reputational harm: Customer trust erosion
