What the Business Problems Document Is

A Business Problems Document is a structured record used to identify, describe, and prioritize operational, financial, legal, or compliance issues facing a company. It documents the problem statement, affected parties, measurable impact, root-cause analysis, and proposed remediation options so stakeholders can evaluate risk and allocate resources. The document is commonly used in project intake, vendor selection, internal audits, and board reporting to create an auditable trail of decisions and corrective actions.

Why a Clear Problems Document Matters

A precise Business Problems Document reduces ambiguity, aligns stakeholders, and creates a defensible record for remediation choices and budget requests. It supports consistent decision-making and helps establish timelines, owners, and measurable success criteria.

Why a Clear Problems Document Matters

Who Typically Prepares and Reviews This Document

Reviewers often include senior leadership and affected department heads; final approvals may require cross-functional sign-off depending on severity and cost.

  • Operations managers and process owners responsible for daily workflows and corrective actions.
  • Legal and compliance teams who assess regulatory exposure and remediation obligations.
