What the Business RAS Template Is

The Business RAS Template is a structured risk assessment and security form organizations use to record identified risks, assigned owners, mitigation plans, and evidence. It standardizes scoring and fields so risks can be compared, aggregated, and reported. The template typically captures risk description, asset mapping, likelihood and impact ratings, mitigation steps, deadlines, responsible parties, status updates, and attachments. Consistent completion helps during audits, regulatory reviews, and internal governance by producing a repeatable record of decisions and remediation activity.

Why Organizations Rely on a Standard RAS Template

The Business RAS Template reduces ambiguity, creates a single source of truth for risk data, and documents accountability. It makes audit evidence easier to collect, shortens assessment cycles, and helps align risk treatment across business units while preserving a consistent record for reporting and compliance purposes.

Why Organizations Rely on a Standard RAS Template

Roles That Typically Complete and Review the Template

Risk owners, compliance personnel, IT and security teams, internal auditors, and business unit leaders commonly complete and review the Business RAS Template to ensure proper tracking.

  • Compliance managers: consolidate regulatory obligations, map controls, and prepare audit-ready evidence packages.
  • IT security teams: identify technical vulnerabilities, assign remediation owners, and verify control status with attached evidence.
  • Business unit leaders: report residual risk, approve timelines, and coordinate corrective actions across teams and vendors.

The template supports cross-functional review cycles and provides a repeatable record for quarterly and annual reporting to leadership.

Representative User Profiles

Compliance Manager

As primary owner, the compliance manager uses the Business RAS Template to map regulatory controls, collect evidence, schedule reviews, and prepare materials for auditors. They coordinate with legal and business units to confirm mitigation sufficiency and update risk ratings after remediation.

IT Security Director

IT security directors complete technical sections, score vulnerabilities, attach scan reports, verify patch timelines, assign remediation tasks, and provide attestations of control effectiveness during quarterly reviews and security governance meetings.

Security and Compliance Controls to Record

Encryption: AES-256 at rest; TLS 1.2/1.3 in transit
Audit Trail: Immutable logs with timestamps and IP addresses
Access Controls: Role-based permissions with SSO/SAML support
HIPAA BAA: Available; BAA required for PHI workflows
21 CFR Controls: Supports required controls for FDA-regulated records
Certifications: SOC 2 Type II, ISO 27001, PCI DSS

Top Legal and Operational Risks to Note

I-9 Violations: Paperwork fines $281–$2,789 (8 CFR §274a.2)
IRS Reporting Errors: $60–$330 per form; IRC §6721
HIPAA Breach: Civil penalties; 45 CFR §164.530(j) impacts retention
Data Exposure: Regulatory fines and remediation costs
Contract Risk: Invalid approvals risk enforcement challenges
Reputational Damage: Loss of customer trust and business impact

Common Preparation Mistakes to Avoid

  • Leaving risk owner or due date blank causes tracking failures and delays in remediation; require fields for owner and concrete deadlines.
  • Using inconsistent likelihood or impact scales across units undermines comparability; adopt a single scoring matrix and include a glossary in the template.
  • Failing to attach logs, tickets, or scan reports prevents auditors from verifying controls; require evidence links or attachments for closed actions.
  • Not verifying signer authority or relying on unauthenticated approvals can weaken enforceability; confirm signer role and authentication method before acceptance.

Step-by-Step: Complete the Business RAS Template

Use the following sequential steps to complete the Business RAS Template accurately and ensure consistent records across teams.

  • 01
Identify Asset: Record the asset and owner with contact details.
  • 02
Describe Risk: Summarize threat, vulnerability, and potential impact.
  • 03
Score Risk: Apply likelihood and impact ratings per matrix.
  • 04

Where to File and Who Receives Completed Templates

Submit completed Business RAS Templates to centralized risk management, the compliance office, and appropriate business unit leads based on your escalation policy.

  • Internal Risk Office: Primary repository for reviews and audit evidence.
  • Compliance Team: Holds regulatory records and coordinates external reporting.
  • Executive Dashboard: Summarized risks for board or senior leadership.

Technical Requirements for Digital Completion and Sharing

Digital completion requires support for PDF and DOCX import, strong signer authentication, and secure integration with enterprise storage and ticketing systems.

  • File Formats: PDF, DOCX, XLSX supported
  • Integrations: Salesforce, NetSuite, Google Workspace, Box
  • Authentication: Email, SMS code, SSO/SAML options

Ensure integrations are configured before rollout, and verify access controls and audit logging across all connected platforms to preserve evidentiary trails.

Key Sections to Include in a Professional Business RAS Template

A complete Business RAS Template should capture essential sections that document context, controls, ownership, and evidence so reviewers can evaluate residual risk and remediation progress.

Risk Summary

Concise description of the identified risk, affected assets, and potential business impact. Include contextual details to help reviewers and auditors, such as incident examples and detection sources, or previous occurrence history.

Risk Scoring

Likelihood and impact scales with numeric values and defined thresholds. Provide a scoring matrix and examples to ensure consistent application across business units and simplify aggregation for dashboards.

Mitigation Plan

Specific actions, timelines, resources, and estimated costs needed to reduce risk. Each mitigation must list responsible party, target completion date, status, and acceptance criteria to confirm effectiveness after implementation.

Ownership & Approvals

Named risk owner, secondary contact, and approver fields. Capture electronic signature or recorded approval timestamps to document authority and satisfy audit requirements during reviews.

Evidence & Links

Attach or link supporting documents such as vulnerability scans, incident tickets, contracts, or test results. Evidence must be retained per retention schedule and referenced in closure notes.

Change Log

Record edits, status changes, reviewer comments, and dates. A versioned change log provides traceability, supports audits, and helps analyze risk trendlines across assessment periods.

Typical Timelines and Processing Expectations

Set clear deadlines for each phase of the assessment lifecycle to ensure remediation momentum and timely reporting to stakeholders.

Initial Assessment Deadline:

Complete risk inventory within 30 days of project start

Mitigation Start:

Begin approved mitigation within agreed timeline, typically 30–90 days

Quarterly Review:

Reassess open risks every quarter and update status accordingly

Annual Audit Submission:

Provide consolidated RAS reports to auditors annually or as required

Incident Reporting Window:

Report breaches promptly per internal policy and applicable law
