What the Business Results Form is and when it’s used

A Business Results Form is a structured report used by organizations to record and communicate periodic performance data. It typically consolidates financial summaries, key performance indicators (KPIs), explanatory commentary, and approvals that support internal decision-making and external reporting. Organizations use this form for quarterly or annual reviews, board packets, investor updates, and regulatory summaries where a consistent, auditable record of results is required to track performance against targets and contractual obligations.

Why a clear, consistent Business Results Form matters

A well-designed Business Results Form standardizes reporting, reduces interpretation errors, and creates an auditable record for governance and compliance.

Why a clear, consistent Business Results Form matters

Who completes and relies on the Business Results Form

Internal teams complete the form and downstream stakeholders use it to make decisions and meet compliance obligations.

  • Finance teams prepare financial rows and reconcile to the general ledger for accuracy and auditability.
  • Operations and product managers supply KPI context and narrative explanations for variances and trends.
  • Executives and board members review summarized results for strategic decisions and investor communications.

Assign clear owners for data entry, review, and sign-off to shorten turnaround and improve accuracy.

Core parts of a professional Business Results Form

Design the form so each component supports transparency: clear identifiers, summarized figures, supporting metrics, narrative context, and an approvals area for accountability.

Header

Company name, reporting period, form version, and preparer contact information for traceability and record control.

Executive Summary

One-paragraph summary highlighting top-line results, major drivers, and material variances that require attention from leadership.

Financial Summary

Standardized revenue, cost, and net income lines tied to accounting codes so reviewers can reconcile with financial systems.

KPI Dashboard

Key operational metrics (conversion, churn, ARPU, backlog) with targets and prior-period comparisons to show trend direction.

Narrative Notes

Context for anomalies, one-time items, and action plans; link or list supporting schedules when relevant to audits.

Approvals

Named approvers, signature/date fields, and version control to establish authorization and an audit trail.

Step-by-step: completing and routing the form

Follow a standard sequence to prepare, validate, approve, and archive each Business Results Form to maintain consistency and auditability.

  • 01
Gather data: Compile financials, KPI extracts, and supporting schedules.
  • 02
Fill fields: Enter values, attach schedules, and add narrative explanations.
  • 03
Internal review: Finance reconciles figures; managers verify narrative accuracy.
  • 04
Sign and archive: Obtain approvals and save final, timestamped copy in records.

Setting up an online completion workflow

Configure a consistent online workflow so submissions follow the same path and required fields are enforced before routing.

Field Configuration
Template Lock core sections and reuse a versioned template for each reporting period.
Conditional Fields Show or hide follow-up questions based on KPI thresholds or checkbox answers.
Authentication Require email or SMS code verification for submitters and approvers.
Notifications Auto-notify reviewers on submission and send reminders for pending approvals.

Typical submission and distribution flow

A reliable sequence reduces delays and creates an auditable completion record for every submission and approval.

  • Upload: Preparer uploads form and attachments to the workflow platform.
  • Assign fields: Place required fields and designate signer order or role-based reviewers.
  • Send to signers: System sends secure signing links or email invites to approvers.
  • Archive: Store final PDF and audit trail in the records repository.

Technical considerations for sharing and signing

Select a platform that supports required authentication, file types, and your preferred distribution channels.

  • Integrations: Salesforce, NetSuite, Google Workspace supported
  • File formats: PDF and DOCX accepted
  • Authentication: Email, SMS, or advanced methods available

Typical timing and processing expectations

Establish clear due dates for each stage and communicate turnaround expectations to preparers and approvers to avoid bottlenecks.

Quarterly submission deadline:

Due within 10 business days after quarter end for initial draft.

Internal review period:

Finance and managers review within 5–7 business days of submission.

Final approval target:

Approvals completed within 10 business days to meet external reporting timelines.

External distribution window:

Investor or regulatory summaries issued within 30 days if required.

Amendment requests:

Corrections should be submitted within 30 days of sign-off.

Key milestones from draft to archival

Track milestones so stakeholders know progress and when records become final and immutable.

01

Draft prepared

Raw data compiled and initial form completed for review.

02

Review cycle

Managers and finance reconcile figures and narrative.

03

Approval/signing

04

Archival

Common preparation errors to avoid

  • Using informal entity names or abbreviations that prevent matching to accounting records and third-party systems.
  • Leaving conditional fields blank when thresholds trigger additional disclosures or supporting schedules.
  • Failing to attach reconciliations and backup schedules, which forces repeated follow-ups during review.
  • Skipping approval metadata (title, date) making it difficult to establish who authorized reported figures.
