What the Business RMM Template Is

A Business RMM Template is a standardized Risk and Remediation Management (RMM) form used to document business risks, controls, mitigation steps, owners, and timelines. It centralizes risk descriptions, likelihood and impact ratings, current controls, planned remediation actions, due dates, and accountable parties. Organizations use the template to track status, escalate unresolved items, and produce an auditable record for internal governance, external review, or regulatory compliance. The template is designed for reuse across projects and departments to ensure consistent risk assessment and remediation tracking.

Why a Consistent RMM Template Matters

Using a consistent Business RMM Template improves traceability, reduces omissions, and creates an auditable record that supports governance and regulatory obligations such as internal control testing and compliance reporting.

Why a Consistent RMM Template Matters

Who Typically Completes the RMM Template

The template is completed by risk owners, compliance teams, IT managers, project leads, and internal audit during risk assessment and remediation planning.

  • Risk & Compliance Teams: Document enterprise risks, controls, and remediation timelines for governance and reporting.
  • IT / Security Operations: Capture technical vulnerabilities, patch schedules, and detection/response owners.
  • Business Unit Managers: Record operational risks, mitigation plans, and resource assignments for business processes.

Completed templates feed periodic status reporting and evidence packages for audits, vendor reviews, or board-level risk summaries.

Essential Sections to Include in the Business RMM Template

A professional RMM template organizes risk data into clear fields so reviewers can quickly assess severity, progress, and ownership across controls and remediation tasks.

Risk Identifier

Unique ID and concise title for each risk to enable cross-referencing and historical tracking across assessments and reporting periods.

Risk Description

Clear narrative of the risk event or condition, including scope, affected systems or processes, and potential business impact if unmitigated.

Likelihood & Impact

Quantitative or qualitative ratings (e.g., Low/Medium/High) with rationale to prioritize remediation effort and resource allocation.

Existing Controls

List of current controls and their status, including testing results or effectiveness notes to justify residual risk.

Remediation Plan

Actionable tasks, assigned owner, target completion date, and required resources to close or mitigate the risk.

Status & Evidence

Current status (Open/In Progress/Closed), completion date, and links or attachments to verification evidence for auditors.

Step-by-Step: Filling and Managing the Template

Follow these steps to populate the template, assign owners, and track remediation through closure.

  • 01
Identify Risks: List each risk separately with a unique ID.
  • 02
Assess Severity: Assign likelihood and impact values with a brief rationale.
  • 03
Assign Owner: Designate an accountable person and enter contact details.
  • 04
Track Progress: Update status, attach evidence, and record closure dates.

Configuring an Online RMM Workflow

Set up a digital workflow so risks move automatically from identification to remediation and audit-ready closure.

Field Configuration
Risk Status Field Dropdown with Open | In Progress | Closed
Owner Assignment Auto-assign based on department or created-by user
Notifications Email or in-app reminders 7 days before due date
Evidence Attachments Allow PDFs, DOCX, and screenshots; limit size per attachment

Typical Routing and Submission Flow

A standard RMM workflow routes entries from reporting to remediation and finally to verification and archival.

  • Report: User submits a new risk entry into the template.
  • Triage: Risk owner reviews and assigns priority and due date.
  • Remediate: Owner performs tasks and uploads evidence of completion.
  • Verify: Compliance or audit team validates closure and archives record.

Platform and Integration Considerations

Choose a platform that supports secure storage, audit trails, and integrations with core systems.

  • File Formats: PDF, DOCX, XLSX supported
  • Integrations: Salesforce, NetSuite, Google Workspace
  • Authentication: SSO and multifactor supported

Integrations reduce manual transfer, enable automated owner assignments, and keep evidence linked to the original risk record for auditors.

Key Dates and Timing Expectations

Set and monitor dates to ensure remediation meets internal SLAs and external reporting requirements.

Initial Assessment Due:

Complete within 5 business days of risk report

Remediation Target:

Owner sets target date in MM/DD/YYYY format

Verification Window:

Compliance verifies within 10 business days after reported closure

Audit Evidence Requirement:

Attach supporting evidence at closure; retain per retention policy

Periodic Review:

Reassess residual risk quarterly or after material change

Notarization and Witness Steps (If Required)

Some remediation agreements or vendor attestations may require notarization or witness signatures depending on state law or contract terms.

01

Determine Requirement

Check governing contract or state statute for notarization need

02

Select Notarization Type

In-person notary or Remote Online Notarization (if permitted)

03

Gather ID

Signer provides government ID or uses approved RON identity method

04

Record Session

RON requires audio-video and journal retention in many states

05

Witness Count

Follow state-specific witness rules for attestations

06

Attach Notary Proof

Include notary acknowledgment or RON certificate in record

07

Confirm Retention

Store notarized documents per retention policy

08

Legal Review

When in doubt, obtain counsel for atypical signatures

Common Preparation Mistakes to Avoid

  • Leaving target dates blank creates open-ended obligations and delays escalation.
  • Using inconsistent severity scales makes prioritization and trend analysis unreliable.
  • Failing to attach verification evidence forces auditors to request follow-up documentation.
  • Recording owner names inconsistently delays remediation and complicates accountability tracking.

Consequences of Incomplete or Incorrect RMM Records

Regulatory Exposure: Increased enforcement risk under applicable statutes
Contract Breach: May trigger vendor penalties or indemnities
Audit Findings: Internal or external audit exceptions
Operational Loss: Unaddressed risk can cause outages or loss
Reputational Harm: Public disclosure can damage trust
Financial Penalties: Fines or remediation costs may apply

Security and Compliance Details to Record

Encryption: TLS 1.2/1.3 in transit; AES-256 at rest
Audit Trail: Timestamps, IP address, and action history
Access Controls: Role-based permissions and SSO
BAA Availability: HIPAA Business Associate Agreement when required
Certifications: SOC 2 Type II and ISO 27001 compliance
Retention Controls: WORM or immutable storage options available
