What the Business Services Cardin is and where it fits

The Business Services Cardin is a standardized intake and engagement document used by service providers to record client identity, scope of work, pricing, billing terms, and signature authorization. It serves as a concise cover agreement that can be attached to contracts, statements of work, or invoices. The Cardin is designed for electronic completion and signing and typically includes fields for effective date, governing state, attachments, and an auditable signature block to create a clear record of mutual expectations before work begins.

Why using a Cardin benefits client intake and recordkeeping

A Business Services Cardin standardizes intake data, reduces manual errors, and creates a consistent, auditable record of client acceptance. When completed with an e-signature workflow that meets ESIGN and UETA criteria, it supports enforceability and reliable retention of engagement terms.

Why using a Cardin benefits client intake and recordkeeping

Who typically completes and relies on the Business Services Cardin

Service providers, account managers, and client onboarding teams commonly complete the Business Services Cardin at engagement start.

  • Professional services firms (consulting, marketing, IT) use it to capture scope, deliverables, and agreed pricing.
  • In-house procurement and vendor management teams use Cardins for standardized supplier onboarding and contract referencing.
  • Healthcare and finance users require HIPAA or recordkeeping notes; include compliance fields when relevant.

Completed Cardins are retained with client files and shared with billing, legal, and delivery teams as part of the project record.

Core elements to include in a professional Cardin

A well-formed Cardin organizes client data, engagement terms, fees, deliverables, signatures, and linked documents for consistent intake and downstream processing.

Client Details

Enter legal entity or full individual name, primary contact, phone, email, and billing address. Accurate client details are required for tax forms and payment processing, and to match identity for signatures.

Scope

Describe services in specific terms, include deliverables, milestones, deadlines, and any exclusions. Avoid vague language; specificity reduces disputes, clarifies billing triggers, and supports accurate resource planning.

Pricing

List fixed fees or hourly rates, invoicing schedule, late payment terms, and accepted payment methods. State whether taxes or expenses are billed separately to prevent misunderstandings.

Terms

Record effective date, term length, renewal conditions, termination rights, governing law, dispute resolution, and administrative notice requirements to support enforceability and clarity.

Attachments

List required attachments such as SOWs, insurance certificates, proofs of qualification, or compliance attestations. Naming conventions and version numbers avoid mismatches during review and archives.

Signatures

Provide signature blocks for authorized representatives, include printed name, title, signature line, and date for each party. Add witness or notary lines where state or industry rules require them.

Common mistakes to avoid when preparing the Cardin

  • Vague scope or deliverables invite disputes; failing to list exclusions or milestones often leads to payment delays and scope creep.
  • Not attaching required exhibits or certificates (insurance, licenses) delays onboarding and may breach contract preconditions for starting work.
  • Initials in place of required signatures, unsigned pages, or unsigned amendments can render the Cardin unenforceable in disputes.

Consequences of incorrect or incomplete Cardins

Tax Filing Errors: Incorrect name/TIN triggers IRS penalties
1099 Backup: Missing TIN can cause 24% backup withholding
Contract Disputes: Ambiguity increases litigation risk
Regulatory Fines: HIPAA or SEC breaches risk fines
Payment Delays: Unclear terms delay invoicing
