What the Business Services CHS Is and when it’s used

The Business Services CHS is a standardized corporate services checklist and client handling sheet used to collect essential business, contact, and authorization details for service engagements. It typically captures party names, legal entity type, tax identifiers, service descriptions, effective dates, billing instructions, and delegated authority. Organizations use this document to centralize onboarding information, confirm responsible signers, and create a consistent record that supports billing, compliance checks, and downstream contracts. Completing a clear, accurate CHS reduces rework, avoids duplicate requests for data, and provides a durable record for audits and regulatory reviews.

Why an accurate Business Services CHS matters

A complete CHS streamlines onboarding by ensuring all parties, billing details, and authorization data are recorded once and reused across workflows. It reduces downstream errors in contracting, invoicing, and regulatory reporting while improving consistency across teams.

Why an accurate Business Services CHS matters

Who typically completes the Business Services CHS

Operational staff, account managers, and client onboarding teams usually prepare the CHS before service commencement.

  • Client onboarding specialists and account managers who collect and verify client data.
  • Finance or billing teams that need tax IDs, billing addresses, and payment terms.
  • Legal and compliance reviewers who confirm signatory authority and regulatory handling.

Legal, finance, and compliance teams review completed CHS records to confirm authority, tax treatment, and retention obligations prior to contract execution.

Step-by-step: completing the Business Services CHS

Follow these steps to collect, verify, and preserve the CHS so it can be relied on for contracting and billing.

  • 01
Collect: Gather legal name, EIN/TIN, billing address, and contact details.
  • 02
Verify: Confirm identity and authority via ID or corporate resolution.
  • 03
Record: Enter information into the CHS template and attach supporting documents.
  • 04
Route: Send for signatures and archive the completed CHS with audit evidence.

Typical electronic workflow for the Business Services CHS

A common digital flow reduces manual handoffs: upload, tag fields, assign signers, authenticate, and store the completed record with an audit trail.

  • Upload Document: Add the CHS PDF or DOCX to the signing platform.
  • Place Fields: Insert signature, date, and data fields for each party.
  • Authenticate Signers: Use email link, SMS code, or stronger methods as required.
  • Capture Audit Trail: Save timestamps, IP addresses, and completion certificates.

Recommended digital workflow settings for Business Services CHS

Configure your e-signature workflow to match the CHS process: signer order, authentication, template reuse, and retention settings.

Platform and file requirements for digital CHS processing

Use a signing platform that supports standard document formats and enterprise integrations to keep CHS data consistent.

  • File Formats: PDF, DOCX, HTML, Excel
  • Integrations: Salesforce, NetSuite, Google Workspace
  • Security Layers: TLS in transit, AES-256 at rest

Core elements to include in a professional Business Services CHS

A well-structured CHS collects identity, authority, financial, and scope details so downstream teams can act without repeated verification or requests.

Identity

Full legal name, DBA if applicable, entity type, and government identifiers to ensure correct tax and legal treatment for the engagement.

Authorization

Name and title of authorized signer, proof of authority such as corporate resolution or power of attorney, and any delegated limits.

Scope

Concise description of services, deliverables, exclusions, and milestones so contracting and billing align with expectations.

Billing

Billing address, payment terms, invoicing contact, tax treatment, and preferred payment method to prevent delays.

Compliance

Regulatory flags (HIPAA, FERPA, export controls), required addenda, and any special handling instructions for protected data.

Attachments

Supporting documents such as W-9, insurance certificates, licenses, and corporate formation records that validate CHS entries.

Security and compliance items to record with the CHS

Encryption: TLS 1.2/1.3 in transit
Data at rest: AES-256 encryption
Certifications: SOC 2 Type II available
Privacy: GDPR and CCPA compliant
Healthcare: HIPAA BAA required
FDA-ready: 21 CFR Part 11 support

Key filing and processing deadlines that affect CHS data

Certain CHS entries inform tax and employment filings. Record dates and identifiers promptly to meet statutory deadlines and avoid penalties.

W-9 Provision Timing:

No filing deadline; provide on payer request

1099-NEC Deadline:

Recipient and IRS due Jan 31 each year

Form 1040 Deadline:

April 15 for individual returns

I-9 Retention:

Keep 3 years after hire or 1 year after termination

FBAR Filing:

April 15 with automatic extension to Oct 15

Common mistakes to avoid when preparing the CHS

  • Using a trade name instead of the legal entity name, which can cause tax reporting errors and banking mismatches.
  • Failing to confirm signer authority, leading to unenforceable agreements or the need for ratification by corporate officers.
  • Omitting or mistyping tax identifiers, which can trigger backup withholding or penalties under IRC rules.
  • Not preserving an audit trail for electronic signatures, complicating dispute resolution or regulatory review.

Penalties and legal risks tied to incorrect CHS data

1099 Penalties: $60/$130/$330 per form (IRC §6721)
Intentional Disregard: $660+ per form, no cap (IRC §6721)
I-9 Violations: $281–$2,789 per violation (DHS guidance)
Backup Withholding: 24% withholding for missing/incorrect TIN
HIPAA Fines: Civil penalties for PHI mishandling
Contract Risk: Invalid signature or authority may void agreements
