What the Business Services RA and SSOW Document Is

The Business Services RA and SSOW Document combines a risk assessment (RA) with a Safe System of Work (SSOW) procedure to identify hazards, evaluate risks, and prescribe controls and sequential steps for safe task execution. It documents affected parties, defined controls, required personal protective equipment, isolation procedures, and emergency responses. The document is used to plan work, communicate duties, and demonstrate due diligence to regulators, clients, and auditors. When complete it creates a traceable record of risk decisions, mitigations, and signatory approvals for each task or project.

Why this Document Matters for Business Services

A combined RA and SSOW clarifies responsibility, reduces workplace incidents, and supports regulatory compliance by documenting hazard identification, control selection, and required checks. It also serves as evidence of a systematic safety process when reviewed by internal auditors or external regulators.

Why this Document Matters for Business Services

Typical Users and Stakeholders

Several roles are responsible for preparing, reviewing, and approving an RA and SSOW depending on company size and project scope.

  • Site managers and supervisors who oversee task execution and enforce controls on location.
  • Health and safety officers who perform hazard analysis, document control, and training records.
  • Contractors and subcontractors who must follow the SSOW and confirm understanding before starting work.

Effective use requires coordination among operational, safety, and contract teams to ensure the document is practical, current, and signed by authorized personnel.

Core Components to Include in a Professional RA and SSOW

A complete RA and SSOW groups information into discrete, actionable sections so teams can follow controls consistently and auditors can verify due diligence.

Scope

Define the task, location, equipment, and personnel covered so the controls apply only to the intended work and avoid ambiguity in responsibilities.

Hazard Identification

List each potential hazard with a concise description, expected cause, and the specific activity or condition that creates the risk exposure.

Risk Assessment

Assign likelihood and severity ratings, record risk scores, and prioritize controls so mitigation choices are documented and defensible.

Control Measures

Specify elimination, substitution, engineering, administrative controls, and PPE required; include testing, isolation procedures, and lockout/tagout steps.

Safe System Steps

Detail a step-by-step work sequence, pre-start checks, handover procedures, and contingency actions to ensure consistent execution.

Sign-off & Review

Include fields for preparer, reviewer, authorized approver, and review frequency with dates to maintain an auditable trail of responsibility.

Essential Fields to Capture

Document Title: RA and SSOW
Effective Date: MM/DD/YYYY
Location: Site name and address
Task Owner: Name and role
Controls Summary: Primary mitigations
Signatures: Preparer, reviewer, approver

Step-by-Step: Completing an RA and SSOW

Follow these sequential steps to create a clear, compliant document that supports safe execution and auditability.

  • 01
Identify Task: Describe scope and boundaries before analysis.
  • 02
List Hazards: Capture all foreseeable hazards linked to the task.
  • 03
Assess Risks: Apply likelihood and severity scoring to prioritize actions.
  • 04
Define Controls: Record specific measures and sign-offs required.

How to Configure an Online RA and SSOW Workflow

Set workflow fields and routing to match your operational approvals and retention policies before issuing the document for signatures.

Field Configuration
Preparer Field Required; auto-fill from HR directory
Reviewer Field Role-based routing to safety officer
Approver Field Require SSO-authenticated approval
Retention Tag Set retention class and export format

Where to Send and File the Completed Document

A typical routing path ensures sign-off, distribution, and archival with a verifiable audit trail.

  • Send to Reviewer: Route digitally for technical review and comments.
  • Approve and Sign: Authorized approver executes final sign-off.
  • Distribute Copies: Send to site team and contractor contacts.
  • Archive: Store in records system with retention tags.

Distribution and eSubmission Options

Choose delivery channels and authentication based on risk level and applicable regulations.

  • Email Distribution: Suitable for low-risk documents; include audit trail metadata.
  • Secure Link: Use one-time links with optional SMS codes for stronger verification.
  • Integration: Connect to systems like NetSuite, Salesforce, or Procore for automatic routing.

Configure authentication strength and retention settings according to internal policy and any regulatory requirements before sharing.

Key Timing: Reviews, Renewals, and Deadlines

Track scheduled reviews and timing triggers to keep controls current and maintain compliance records.

Initial Approval Deadline:

Complete authorizations before work start.

Periodic Review:

Review at least annually or after incident.

Pre-Start Checks:

Conduct site checks each shift or before high-risk tasks.

Training Completion:

Ensure assigned personnel complete required training prior to work.

Record Retention Start:

Retention begins on effective date of document.

Common Mistakes to Avoid

  • Omitting a clear scope which leads to inconsistent application of controls across sites or teams.
  • Using vague controls like 'use caution' instead of specifying concrete, verifiable measures and PPE.
  • Failing to route for proper approvals so the document lacks an authorized signatory and cannot be enforced.
  • Not updating the document after process changes or incidents, which undermines its effectiveness and auditability.

Consequences of an Incorrect or Incomplete Document

Regulatory Fines: OSHA citations and fines
Contract Liability: Breach claims and indemnity exposure
Worker Injury: Increased injury risk and workers' comp costs
Operational Delay: Work stoppage or rework
Insurance Impact: Higher premiums or denied claims
Reputational Damage: Loss of client trust

How an RA and SSOW Differs from Related Documents

Criteria RA & SSOW SOP
Primary Purpose risk control process consistency
Legal Weight high for safety compliance moderate
Frequency Updated project or change-driven process-driven
Typical Use high-risk tasks routine operations
