What the Business Services SDQ Is and when it's used

The Business Services SDQ (Service Delivery Questionnaire) is a standardized vendor-facing questionnaire used to collect operational, compliance, and service-level information during procurement and onboarding. It typically asks about company details, service scope, security controls, subcontractors, performance metrics, insurance, and continuity planning. Organizations use the SDQ to evaluate supplier capability, compare responses across bidders, and document due diligence. Completed SDQs form part of the contracting record and inform contract terms, SLAs, and risk assessments; they may be required before final approval or payment authorization.

Why a clear SDQ matters for procurement and compliance

A well-prepared Business Services SDQ provides consistent data for vendor selection, reduces follow-up questions, and documents risk controls that feed contract terms and audits.

Why a clear SDQ matters for procurement and compliance

Who typically completes and reviews a Business Services SDQ

Different teams interact with the SDQ at separate stages of procurement and oversight.

  • Procurement teams — collect responses, score supplier capability, and manage vendor selection workflows.
  • Security and compliance — verify security controls, request evidence, and assess regulatory gaps.
  • Contract managers — convert SDQ responses into contract clauses and SLA commitments.

Cross-functional review of SDQ responses reduces negotiation cycles and supports auditability across finance, legal, and IT.

Core sections to include in a professional Business Services SDQ

A complete SDQ groups information into consistent sections so evaluators can compare vendors quickly and trace answers back to evidence or documents.

Company identity

Legal name, DBA, EIN, corporate address, primary contact, and corporate structure for verification and tax purposes.

Service scope

Detailed description of services offered, deliverables, hours of operation, geographic coverage, and any subcontracted activities.

Security controls

Information on encryption, access control, vulnerability management, incident response, and certifications such as SOC 2 or ISO 27001.

Compliance and privacy

Regulatory status including HIPAA, FERPA, PCI, data residency, and whether the vendor will sign a BAA where required.

Business continuity

Disaster recovery plans, recovery time objectives, backup frequency, and recent DR test results.

Liability and insurance

General liability, cyber liability, limits, and certificate of insurance details that affect contract risk allocation.

Essential fields to capture in the SDQ

Legal Entity: Registered name
Tax ID: EIN or SSN
Primary Contact: Name and email
Service Description: Scope summary
Regulatory Status: HIPAA/PCI/Other
Insurance Limits: Policy amounts

Step-by-step: completing a Business Services SDQ

Follow these sequential steps to collect, validate, and store SDQ responses for procurement and legal review.

  • 01
Prepare the template: Assemble standard sections and required attachments.
  • 02
Send to vendor: Distribute via secure link or portal with a clear deadline.
  • 03
Review responses: Verify factual fields and request missing evidence.
  • 04
Record outcome: Attach SDQ to contract and retention repository.

Configuring an online SDQ workflow

Map online settings to your procurement and compliance needs before distributing the SDQ to vendors.

Field Configuration
Authentication Email link | SMS code | KBA
Attachments Require evidence uploads (PDF, DOCX)
Conditional Questions Show only when relevant to reduce friction
Audit Trail Enable timestamps, IP, and action logs

Distribution channels and technical requirements

Choose secure delivery and storage channels aligned with your security policy and procurement systems.

  • Email links: Low friction; use one-time links and link expiry to reduce unauthorized access
  • Portal upload: Centralizes responses and attachments for auditability
  • Integrations: Connectors (CRM/ERP) streamline vendor record creation and contract linkage

Ensure chosen channels support required encryption, evidence retention, and integration with procurement or contract management systems.

Typical SDQ lifecycle from request through contract

A concise process diagram helps stakeholders understand handoffs and timing for SDQ completion and review.

  • Initiate Request: Procurement triggers SDQ and assigns reviewers.
  • Vendor Responds: Vendor returns completed SDQ with attachments.
  • Validation: Security and legal teams verify claims.
  • Contract Integration: Finalized responses inform contract terms and storage.

Typical timelines and processing expectations

Establish clear deadlines for vendors and internal reviewers to avoid procurement delays.

Vendor response window:

Commonly 7–14 calendar days depending on complexity.

Internal review:

Allow 3–7 business days for security and legal checks.

Follow-up requests:

Send clarifying questions within 48–72 hours of initial review.

Final approval:

Typically within 5 business days after all evidence is provided.

Contract execution:

Sign within agreed procurement SLA once SDQ conditions met.

Risks and potential consequences of incomplete or inaccurate SDQs

Contract delays: Negotiation stalls
Regulatory exposure: Noncompliance fines
Operational failure: Unvetted subcontractors
Insurance gaps: Claims denied
Reputational harm: Customer trust loss
Financial loss: Remediation costs

Real-world examples of SDQ use

Examples illustrate how organizations apply SDQs during vendor selection and contract management.

Optica Ventures

Optica required a standardized SDQ for all service providers to reduce onboarding time

  • 3-week onboarding reduced to 4 business days
  • The SDQ centralized evidence and reduced follow-up requests, streamlining approvals and vendor setup.

Xerox / NetSuite

Xerox integrated SDQ responses with NetSuite for automated vendor records

  • Integration removed manual data entry
  • This enabled faster contract issuance and clearer audit trails between procurement and finance systems.

Practical tips to complete SDQs accurately and efficiently

These practices reduce review cycles and support consistent evaluation across vendors.

Standardize templates
Use a single SDQ template with controlled conditional questions to ensure parity across bidders.
Require evidence
Request specific documents (certificates, policy excerpts) rather than generic attestations to speed validation.
Use electronic workflows
Enable audit trails, attachment validation, and conditional fields to reduce manual errors.
Assign reviewers
Designate security, legal, and procurement reviewers to avoid bottlenecks and conflicting feedback.
