What the Business Services SO618 Is and when it applies

The Business Services SO618 is a standardized request and authorization form used to specify, approve, and document contracted business services between organizations. It records the parties, scope of work, deliverables, pricing or rates, payment terms, effective dates, and required approvals. When completed and signed, the SO618 creates an administrative record used for procurement, invoicing, and internal audit. The form is commonly used by operations, procurement, and finance teams to ensure consistent service intake and to support compliance and retention obligations under applicable U.S. law.

Why using a clear SO618 matters for operations and compliance

A complete SO618 reduces ambiguity about scope, cost, and responsibilities, supports consistent approvals, and creates an auditable record for financial and regulatory reviews. Properly executed forms protect both parties and support downstream invoicing and reporting processes.

Why using a clear SO618 matters for operations and compliance

Primary users and stakeholders for the SO618

The SO618 is used by requesters, approvers, and contract administrators across functional teams.

  • Requesters — Operational managers who define scope and provide service requirements and timelines.
  • Approvers — Finance or procurement approvers who confirm budget, vendor selection, and payment terms.
  • Administrators — Contract or legal teams who verify compliance, signatory authority, and retention obligations.

Each role has distinct responsibilities: request creation, budget approval, contract review, and final execution.

Typical signatories and their responsibilities

Requester - Operations

Operations managers complete the service description, timeline, and required deliverables then verify operational feasibility and resource allocation before routing for approval.

Approver - Finance

Finance reviewers confirm budget availability, payment terms, tax treatment, and whether vendor onboarding requirements are met prior to authorizing the form.

Core sections to include in a professional SO618

A well-constructed SO618 groups essential information into discrete sections so reviewers can quickly verify scope, cost, and authority.

Follow a consistent sequence to reduce errors and ensure approvals and records are preserved.

  • 01
Prepare the form: Upload or open the current SO618 version and enter all required fields.
  • 02
Attach exhibits: Add statements of work, pricing spreadsheets, and insurance proofs as needed.
  • 03
Internal review: Route to procurement and finance for budget and tax validation.
  • 04
Obtain signatures: Collect required signatures in the order specified and record execution dates.

Configuring an online completion workflow

Define field behavior, signer order, and authentication to mirror your internal approval process before sending for signature.

Field | Configuration Name | Validation | Required
Requester Name Auto-fill from user profile | Required
Effective Date Date picker MM/DD/YYYY | Required
Amount Numeric format two decimals | Required
Signers Sequential routing with email/SMS authentication | Required

Where the completed SO618 should go next

Routing depends on organizational policy; capture a copy in finance and in the contract repository for audit.

  • Procurement: Receives executed form for vendor onboarding and PO creation.
  • Finance: Uses form to match invoices and initiate payment.
  • Contract Repository: Stores final executed document for retention and audit.
  • Vendor: Receives signed copy as authorization to commence work.

Digital submission and file format considerations

Use platform features that preserve field data, provide an audit trail, and export signed records in standard formats.

  • File Formats: PDF or DOCX preferred for preservation
  • Integrations: Support for ERP and cloud storage
  • Audit Trail: Capture timestamps, IP, and signer metadata

Typical timelines and processing expectations

Set clear internal deadlines to avoid service delays; communicate expected review windows to vendors and internal approvers.

Initial request to approval:

Allow 5–10 business days for full internal review

Vendor acceptance:

Vendors typically confirm within 3–5 business days

Payment processing:

Payment often follows invoice match within 30 days

Document retention start:

Retention clock starts on effective or execution date

Expedited processing:

Flag as urgent when approval needed under 48 hours

Common preparation mistakes to avoid

  • Incomplete scope descriptions that lead to disputes over deliverables and extra cost claims later in the engagement.
  • Mismatched payee names or incorrect EIN/TIN entries that delay payments or trigger backup withholding tax requirements.
  • Missing approvals or wrong signer authority that render the document unenforceable or require re-execution.
  • Attaching unsigned or draft exhibits instead of final, signed statements of work which creates ambiguity in obligations.

Consequences of errors or missing information

Delayed Payment: Cashflow disruption for vendor
Tax Withholding: Backup withholding 24% may apply
Contract Invalidity: Improper authority can void agreement
Regulatory Exposure: Noncompliance with sector rules possible
Audit Findings: Recordkeeping gaps noted in audits
Late Fees: Penalties per payment terms
