What a Business Submission Sheet Is

A Business Submission Sheet is a one-page document used to gather standardized information about a commercial request, proposal, or application. It typically summarizes the submitting entity, contact details, submission date, scope or purpose, and key attachments so reviewers can triage and route materials efficiently. Organizations use the sheet to create an auditable record of what was submitted, when, and by whom, reducing back-and-forth clarifications and centralizing essential metadata for processing and filing.

Why standardizing submissions matters

A concise, consistent Business Submission Sheet speeds review cycles, reduces errors, and preserves a clear record of intent and supporting documents for compliance and auditability.

Why standardizing submissions matters

Who typically completes a Business Submission Sheet

Business Submission Sheets are used by internal teams and external partners to present requests in a consistent format for evaluation and recordkeeping.

  • Project managers and operations staff who need a single record to route requests to reviewers and approvers.
  • Sales and account teams submitting proposals, pricing, or contract change requests to procurement or legal.
  • Vendors and third parties delivering qualifications, invoices, or compliance documents alongside attachments.

The sheet helps reviewers compare submissions quickly and establishes who is responsible for the content and follow-up.

Step-by-step: completing the Business Submission Sheet

Follow these steps in order to prepare a complete, reviewer-ready submission.

  • 01
Prepare files: Assemble all supporting documents and name files clearly.
  • 02
Populate fields: Complete every required field using the formats above.
  • 03
Authorize: Obtain required signatures from authorized persons.
  • 04
Route: Send to the correct reviewer or upload to the intake system.

Essential elements to include on a professional sheet

A well-constructed Business Submission Sheet contains consistent sections so reviewers can find and verify critical information at a glance.

Header

Document title, version number, and a unique submission identifier to make tracking and retrieval unambiguous across systems and audits.

Submission Summary

A short description of the request or deliverable that orients reviewers without reading full attachments, including scope and requested action.

Parties

Clear identification of submitting party and recipient, including legal name, address, and primary contact for follow-up and service of notices.

Attachments

An indexed list of included files with file types and version numbers so reviewers can validate they are working from the intended materials.

Signatures

Signature block with printed name, title, date, and method (wet sign or eSignature) identifying the authorized signer for the submission.

Routing Notes

Fields for reviewer assignment, priority, and required deadlines that signal the processing path and service-level expectations.

Security and compliance summary fields

Access Level: Define who can view
Confidentiality: Classify data sensitivity
HIPAA Flag: Mark if PHI present
Retention Tag: Assign retention class
Audit Trail: Record submission events
Encryption: At-rest and in-transit

Consequences of incomplete or incorrect sheets

Processing Delays: Missed deadlines
Regulatory Risk: Noncompliance exposure
Financial Penalties: Filing fines possible
Contract Rejection: Proposals may be voided
Audit Findings: Recordkeeping gaps cited
Tax Impact: Incorrect reporting

Common pitfalls to avoid

  • Submitting incomplete attachments or outdated versions that force manual follow-up and rework by reviewers, increasing turnaround times and error risk.
  • Using informal or nonstandard naming conventions for files, which leads to misfiling and difficulty locating the controlling document during audits.
  • Failing to record the authorized signer or using an unauthorized signature that may render the submission nonbinding or subject to rejection.
  • Not indicating confidentiality or PHI flags, which can lead to improper handling and potential HIPAA violations or data exposure.

Typical routing and review workflow

Submissions normally follow a linear intake, review, approval, and archive flow; document each step and responsible party.

  • Intake: Submit sheet and attachments to intake queue.
  • Triage: Reviewer checks completeness and classifies priority.
  • Approval: Designated approver accepts or requests changes.
  • Archive: Store final package in records system.

Configuring digital workflows for submissions

Map each form field to a workflow action to automate routing, approvals, and archival.

Field Configuration
Submission Type Route to reviewer queue
Priority Trigger SLA timer
Attachments Require PDF format
Signature Lock and route for eSign

Digital delivery and signing requirements

Electronic workflows should support secure uploads, configurable fields, and auditable signatures before routing to reviewers.

  • File formats: PDF, DOCX, XLSX supported
  • Authentication: Email, SMS, or stronger MFA
  • Integrations: CRM and document storage

Align platform capabilities with security, retention, and integration needs to maintain chain-of-custody and streamline processing.
