What the Business Tax Form SS-4 Is

Business Tax Form SS-4 is the Internal Revenue Service (IRS) application used to request an Employer Identification Number (EIN) for a business entity, sole proprietorship, estate, trust, partnership, or nonprofit. The EIN is the federal taxpayer identification number used for tax reporting, payroll, banking, and federal tax filings. The SS-4 collects entity legal name, trade name, responsible party information, reason for applying, and business start date. The form can be submitted online, by fax, or by mail; an EIN is issued immediately when the IRS online application is completed successfully.

Why an EIN via Form SS-4 Matters

Applying with Form SS-4 secures a distinct federal EIN required for tax reporting, payroll withholding, banking, and vendor relationships. An EIN separates business and personal tax liabilities, enables proper employer tax accounts, and is often required to open business bank accounts and hire employees.

Why an EIN via Form SS-4 Matters

Who Typically Files Form SS-4

Common filers include small business owners, corporations, partnerships, estates, trusts, and nonprofits applying to obtain an EIN.

  • New LLCs and corporations that need an EIN for tax filings and bank accounts
  • Sole proprietors hiring employees or opening business bank accounts for payroll
  • Estates and trusts reporting tax matters and needing distinct taxpayer identification

Authorized representatives, such as CPAs or attorneys, can prepare or file Form SS-4 on behalf of an entity when properly appointed.

Key Parts of Form SS-4 to Complete

Understand the SS-4 structure to provide accurate identifiers, responsible party details, and the reason for requesting an EIN before submission.

Applicant Name

Enter the entity's legal name exactly as on formation documents; inconsistencies can delay IRS processing and complicate later tax filings, banking, and employer account registration.

Trade Name

Provide the DBA or trade name when it differs from the legal name; financial institutions and vendors use this for accounts and contracts. Omitting a DBA may cause mismatched records and administrative delays with state agencies.

Responsible Party

List the individual or entity who controls, manages, or directs funds and assets; include their SSN, ITIN, or EIN where required. Accurate responsible party details are used by the IRS to verify identity and assign accountability for tax obligations.

Reason for Applying

Select the correct reason code—starting a new business, hiring employees, restructuring, or banking requirements. The choice affects follow-up registration obligations, payroll account setup, and whether additional forms or state registrations are necessary.

Business Start Date

Enter the date business began or will begin operations in MM/DD/YYYY format if known; this date can affect tax year selection, payroll reporting, and the timing of state registrations.

Mailing Address

Provide a complete mailing address including street, city, state, and ZIP. Use a physical address when possible; P.O. boxes can be acceptable for mail but may not be sufficient for certain state filings.
