What the Business TELUS Document Is

The Business TELUS Document is a formal account and authorization form used to establish, update, or verify commercial service relationships between a business and a carrier or service provider. It typically collects the legal entity name, federal tax ID or EIN, authorized signatories, billing and service addresses, account numbers, and the specific services requested. When completed and retained according to federal and industry rules, the executed document becomes an evidentiary record of authorization and instructions for provisioning, billing, and contract administration under ESIGN and applicable state electronic-transaction laws.

Why a Correctly Completed Form Matters

A complete Business TELUS Document reduces billing disputes, speeds provisioning, and creates a clear legal record of authorization. It supports regulatory compliance under ESIGN (15 U.S.C. §7001) and UETA where applicable, and preserves auditability for internal controls and audits.

Why a Correctly Completed Form Matters

Who Typically Prepares and Signs This Document

Several business roles interact with this form depending on its purpose; the list below shows common preparers and signers.

  • Finance and Accounts Payable: Prepares billing details, verifies EIN, and reconciles payment authorization.
  • IT or Telecom Manager: Provides technical account identifiers, MACs, service locations, and porting instructions.
  • Authorized Officer or Contract Manager: Signs for company authority and accepts terms on behalf of the legal entity.

Assign the task to the role that controls billing and contractual authority to avoid processing delays.
