What the Business Valuation Change Form Is

The Business Valuation Change Form documents a revision to an existing valuation, capturing changed assumptions, updated financials, and the effective date for the new value. Organizations use it to record agreed adjustments for tax reporting, shareholder transactions, buy-sell agreements, financing, and internal controls. The form should identify the parties, describe the prior valuation, state the changed items and methodology, and include signatures and dates. When executed correctly it creates an auditable record that supports later reliance by accountants, counsel, or regulators.

Why a Formal Change Form Matters

A formal Business Valuation Change Form reduces dispute risk by documenting what changed, why it changed, and when the new valuation takes effect. It creates a single authoritative record for tax, corporate governance, and transactional purposes.

Why a Formal Change Form Matters

Typical Users and Signers

In many organizations multiple roles must review and approve the form to ensure accuracy and legal authority.

  • CFOs and Finance Leads: prepare, verify numbers, and approve changes for reporting and covenants.
  • Valuation Specialists: document methodology, assumptions, and revised calculations for audit trails.
  • Legal Counsel or Corporate Secretary: confirm authority, governance compliance, and execution language.

Core Elements Inside a Professional Change Form
