What the Business Value Request Form Is

The Business Value Request Form is a standardized internal document used to request analysis, approval, or funding for projects, initiatives, or vendor engagements. It captures the proposed project's objectives, scope, projected benefits, costs, metrics for success, risks, stakeholders, and required resources. Organizations use the form to create a consistent decision record, enable comparative evaluation, and document expected return on investment or operational impact. The completed form supports budgeting, procurement, and executive review processes and becomes part of the official project documentation retained for compliance and audit purposes.

Why a Standardized Request Form Matters

Use the Business Value Request Form to standardize project evaluation, clarify expected benefits, and record approval rationale. It reduces ad hoc requests, improves comparability across proposals, and provides an auditable trail for budgeting, procurement decisions, and post-implementation performance reviews.

Why a Standardized Request Form Matters

Who Prepares and Reviews These Requests

Typical roles that prepare or review the Business Value Request Form include cross-functional stakeholders from finance, IT, procurement, and business units.

  • Project sponsors and managers who define scope, submit cost estimates, and shepherd approvals through governance.
  • Finance or budget owners who validate cost assumptions, funding sources, and perform ROI calculations for decision makers.
  • Procurement and legal reviewers who confirm vendor terms, compliance, and contracting prerequisites before commitments.

Use these role definitions to route the form correctly and reduce review cycles by ensuring the right reviewers are included from the start.

Primary Roles and Responsibilities

Finance Manager

Manages budget validation, reviews cost assumptions, verifies funding availability, and confirms accounting treatment. Finance managers often run sensitivity analyses and sign off on funding before procurement proceeds, ensuring alignment with fiscal controls.

Project Sponsor

Accountable executive or business owner proposing the initiative. The sponsor defines objectives, secures resources, answers reviewer questions, and accepts responsibility for delivery and performance metrics after approval.

  • 01
Draft Request: Enter project details, scope, objectives, and initial cost estimates.
  • 02
Attach Financials: Upload supporting budgets, ROI calculations, and vendor quotes.
  • 03
Submit for Review: Route to listed approvers in sequence with required documentation.
  • 04

  • Upload Document: Upload completed form and attachments to the platform.
  • Place Fields: Define required inputs and signature blocks.
  • Assign Signers: Specify approvers and authentication method.
  • Capture Audit Trail: Automatic timestamps, IPs, and completion certificates.

Technology Considerations for eSubmission

Core Sections Every Form Should Include

A professional Business Value Request Form includes structured sections for project overview, financials, benefits, risks, implementation plan, and approval routing to support consistent evaluation.

Project Overview

Concise description of objectives, scope, business case, stakeholders, and alignment to strategic priorities. Include the requested timeline and any key dependencies that could affect resource allocation or delivery.

Financial Summary

Summarize total estimated costs, funding source, and breakdown by capital, operating, or one-time expenses. Attach supporting worksheets, assumptions, and sensitivity analysis for reviewer validation and audit evidence.

Benefit Metrics

List measurable outcomes such as annual cost savings, revenue uplift, productivity gains, or risk reduction. State baseline, projected value, and the timeframe for achieving each benefit.

Risk Assessment

Identify top risks, probability and impact estimates, mitigation plans, and residual risk after controls. Note any compliance, legal, or vendor risks that may affect approval or implementation.

Implementation Plan

Provide high-level milestones, resource requirements, key deliverables, and a provisional schedule. Include internal owners, third-party vendors, and handoff points to operational teams.

Approval Routing

Specify required approvers, approval thresholds, and escalation paths. Include delegated authorities and any required procurement or legal reviews before funds are committed.

Best Practices to Reduce Review Time

Practical tips for accurate and efficient completion help prevent delays, improve approvals, and support auditing of project value requests.

Include complete financials and assumptions
Provide detailed worksheets, assumptions, unit costs, sensitivity analyses, and vendor quotes. Clearly identify one-time versus recurring costs and show how benefits were quantified. This detail reduces questions and speeds reviewer validation during budgeting and procurement.
Route to all required approvers early
Identify and include all approvers and subject-matter reviewers in the initial submission to avoid iterative routing. Document delegated authority limits and attach evidence of approvals when appropriate. Early routing reduces rework and shortens overall approval cycles.
Use consistent project names and codes
Match project titles and codes to finance and procurement systems to simplify reconciliation. Include cross-references to purchase requisition numbers, contract IDs, and existing budget line items to avoid duplication and ensure traceable funding.
Keep documentation attached and organized
Attach vendor quotes, schedules, legal approvals, and versioned financial models. Use standardized file names, include a brief attachment index in the form, and confirm files are accessible to reviewers to prevent delays due to missing documents.

Timelines, Deadlines, and Processing Expectations

Key timing and submission expectations for Business Value Request Form processing, approvals, and related procurement or finance reporting are summarized below.

Initial Submission Timeline:

Submit before next budget cycle; complex projects allow 4–8 weeks for evaluation.

Approvals and Escalation:

Routine approvals 3–10 business days; escalate if unresolved after two review cycles.

Budget Cycle Alignment:

Align request with fiscal deadlines to ensure funding availability.

Tax Reporting Impact:

Capital projects may affect depreciation schedules and tax filings.

Record Retention Start:

Retention begins on approval date or effective date noted on form.

Optica Ventures

Optica Ventures streamlined project approvals by combining the Business Value Request Form with digital signatures to reduce manual routing.

  • Faster approvals and clearer financial reporting.
  • By enforcing structured fields and attaching vendor quotes, the company shortened review cycles, reduced back-and-forth clarifications, and created a single source of record for audit purposes.

Fertility Centers

Fertility Centers centralized approvals and ensured compliance by requiring complete financial attachments and signatory accountability on every request.

  • Improved compliance and document traceability.
  • The organization saw fewer missing attachments, clearer approval trails, and faster handoffs to finance and operations, simplifying audits and supporting regulatory reporting without adding administrative burden.

Common Mistakes to Avoid

  • Submitting incomplete cost breakdowns, such as missing operating versus capital distinctions, which slows review and prevents accurate ROI calculations.
  • Omitting required approvals or failing to list delegated authority levels, leading to routing errors and repeated review cycles.
  • Using inconsistent project names or codes that do not match procurement or finance records, causing reconciliation delays.
  • Attaching unsupported assumptions without sensitivity analysis, increasing the likelihood of rejection or requests for additional financial detail.
