What the CACFP Program Signature Page Is

The CACFP Program Signature Page is the attestation page used with Child and Adult Care Food Program (CACFP) claims and related provider documentation. It records the authorized representative's name, title, signature, and date to certify meal counts, enrollment data, and compliance with USDA program requirements. Agencies, sponsors, and providers use this page to confirm accuracy of submitted claims and to document who is responsible for the information. A correctly completed signature page is often required before state agencies will process reimbursement or perform audits.

Why Accurate Signature Pages Matter

A properly completed CACFP Program Signature Page creates an auditable legal attestation that supports reimbursement, demonstrates program compliance, and reduces the risk of claim disallowance or repayment. It establishes who is accountable for submitted meal claims and links those claims to documented meal counts and enrollment records.

Why Accurate Signature Pages Matter

Who Typically Completes the CACFP Signature Page

The CACFP Program Signature Page is completed by parties responsible for submitting or certifying meal claims.

  • Child care providers and home providers who prepare and submit monthly meal claims to their sponsoring organization.
  • Sponsoring organizations (approved CACFP sponsors) certifying accuracy of aggregated claim data on behalf of multiple sites.
  • State agency administrators and auditors who review, accept, or reject claims based on documented attestations.

The completed page supports sponsors and state agencies during review and audit processes.

Step-by-Step: Completing the CACFP Program Signature Page

Follow these sequential steps to complete the page accurately and create a defensible record for claims.

  • 01
Review Records: Confirm meal counts and attendance match source documentation.
  • 02
Identify Signer: Ensure the signer is the authorized representative on file.
  • 03
Complete Fields: Fill in name, title, program, claim period, and totals.
  • 04
Sign & Date: Apply a valid signature and enter the MM/DD/YYYY date.

Core Elements Found on a Professional CACFP Signature Page

A complete signature page groups identification, claim specifics, certification language, signature, and contact details so reviewers can verify and accept claims with confidence.

Certification Statement

Clear attestation language stating the signer certifies the accuracy of meal claims and compliance with USDA CACFP regulations; the statement defines legal accountability.

Authorized Representative

Name and title field identifying the individual legally permitted to certify claims for the provider or sponsor; required for audits.

Program / Site Identifier

Official program or site name and identifying number used by the sponsor and state agency to link the signature to submitted claim data.

Claim Period

Explicit period (month and year or specific dates) covered by the attestation, ensuring the signature applies only to the intended submission.

Totals and Metrics

Key numeric fields such as total breakfasts, lunches, suppers, snacks, and enrolled participants that the signature certifies.

Signature Block

Physical or electronic signature area plus printed name and date, and optional contact phone or email for follow-up verification.

Distribution and Digital Signing Considerations

Choose a platform and configuration that preserves audit trails, secures data at rest and in transit, and meets program rules.

  • File Formats: PDF, DOCX accepted
  • Integrations: Salesforce, NetSuite, Google Workspace
  • Security: TLS in transit; AES-256 at rest

Where to Send the Completed Signature Page

Routing depends on sponsor and state agency procedures; confirm the recipient and acceptable submission method before sending.

  • Sponsor Portal: Upload to the sponsor's claim submission system.
  • State Agency: File through the state's CACFP portal when required.
  • Email Submission: Send to designated sponsor email if permitted.
  • Secure eSign: Use a compliant eSignature platform for audit trails.

Timelines and Processing Expectations

Be aware of sponsor and state deadlines for monthly claim submission and any internal review windows that affect reimbursement timing.

Monthly Claim Deadline:

Varies by sponsor; typically within 15–45 days after month end

Sponsor Review Window:

Allows edits before state submission

State Submission Cutoff:

Set by state agency schedule

Audit Notification:

May occur months after submission

Repayment Timing:

Determined after audit findings

Key Processing Milestones for a Claim

A typical claim moves through preparation, sponsor review, state submission, and either payment or audit; each stage has predictable outcomes.

01

Prepare Claim

Compile meal counts and complete signature page before sponsor cutoff.

02

Sponsor Review

Sponsor verifies totals and attestations, requests corrections if needed.

03

State Submission

Sponsor or provider submits finalized claim to the state agency for reimbursement.

04

Payment or Audit

State issues reimbursement or selects claim for audit and follow-up.

Common Mistakes to Avoid

  • Using an unsigned or undated signature page that leaves the claim uncertified and triggers sponsor rejection or delay.
  • Entering totals that do not match source meal counting records, causing math discrepancies and possible repayment.
  • Submitting a signature from an unauthorized individual rather than the designated representative on file with the sponsor.
  • Failing to attach required supporting documentation such as daily meal attendance sheets, which can prompt audit findings.

Risks and Consequences of Incorrect Signature Pages

Claim Denial: Partial or full rejection
Repayment: Funds recovery may be required
Civil Penalties: Monetary sanctions possible
Program Disqualification: Sponsor or provider risk
Audit Findings: May trigger multi-period review
Reputational Harm: Loss of trust with sponsor
