The California Form 3525 Substitute for Form W-2 and Form 1099-R is a state-specific substitute form employers and payers may provide to recipients that mirrors federal W-2 and 1099-R information for state tax, withholding, and recordkeeping purposes. It consolidates wage, withholding, and distribution details on a California-formatted statement so employers, payroll providers, and plan administrators can meet California reporting practices while still supporting federal filings.

Why a State Substitute Matters for California Reporting

Using a properly completed Form 3525 substitute helps ensure recipients receive the wage and distribution details they need for California returns and withholding reconciliation, and helps employers align state-format reporting with federal records.
