ASSUMPTION OF RISK, RELEASE, AND INDEMNIFICATION

Participant acknowledges that participation in camp activities involves inherent risks, including but not limited to physical injury, illness, disability, property loss, or death. Participant voluntarily assumes all risks associated with participation and agrees to release, waive, discharge, and covenant not to sue Camp Provider, its owners, directors, employees, volunteers, agents, and affiliates (collectively, Released Parties) from any and all liability for any loss, damage, injury, claim, or cause of action arising out of or related to participation, whether caused by the negligence of Released Parties or otherwise, to the fullest extent permitted by law.

Participant (and Parent/Guardian if Participant is a minor) agrees to indemnify, defend, and hold harmless Released Parties from any claim, demand, loss, liability, cost, or expense (including reasonable attorneys' fees) arising out of Participant's actions or breach of this Agreement.

MEDICAL AUTHORIZATION

In the event of illness or injury, Participant authorizes Camp Provider to provide or secure medical treatment deemed necessary. Participant consents to the administration of first aid and emergency medical care and authorizes medical personnel to provide treatment. Participant acknowledges financial responsibility for any medical services rendered.

PHOTO / MEDIA RELEASE

I consent to Camp Provider's use of photographs, video, and other media that may include images of Participant for promotional or educational purposes. Check to consent:

CODE OF CONDUCT

Participant agrees to comply with all camp rules and staff instructions. Camp Provider may remove Participant from activities or send Participant home for behavior that endangers others, violates rules, or disrupts programming. Removal for disciplinary reasons does not relieve Participant's financial obligations unless otherwise determined by Camp Provider in its sole discretion.

CONFIDENTIALITY

All personal information provided in this form will be used solely for camp administration, safety, and health purposes. Parties agree to maintain the confidentiality of sensitive health information except as required to provide care, comply with legal obligations, or respond to emergencies.

GOVERNING LAW; ENTIRE AGREEMENT; SEVERABILITY

This Agreement is governed by the laws of the state in which the camp is operated. This document constitutes the entire agreement between the parties with respect to the subject matter herein and supersedes all prior understandings. If any provision is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.
