What a Campaign Disclosure Form Is and when it applies

A Campaign Disclosure Form is an official record used by federal, state, or local election authorities to report contributions, expenditures, debts, and other financial activity for political campaigns, committees, and ballot measure committees. It typically lists donor names, dates, amounts, purposes of expenditures, and summary reconciliations. Filing frequency and content requirements depend on the jurisdiction and the filer’s status; many authorities now accept electronic submission and signatures in accordance with ESIGN and applicable state electronic-record laws such as UETA or a state ESRA.

Why accurate campaign disclosure matters

Accurate disclosures ensure statutory compliance, reduce audit and enforcement risk, and support public transparency. Properly completed forms also create an auditable record that agencies can reproduce and verify, which helps avoid fines, legal exposure, and reputational harm.

Why accurate campaign disclosure matters

Who commonly prepares and files these forms

Typical filers include candidates, political committees, party organizations, and campaign treasurers responsible for preparing and submitting disclosures.

  • Federal candidates and authorized committees who file with the Federal Election Commission.
  • State ballot committees and candidates filing with state campaign finance or ethics agencies.
  • Party committees, political action committees (PACs), and independent expenditure groups.

Depending on jurisdiction, filings may be prepared in-house by treasurers, by outside compliance vendors, or by counsel; identify the designated filer before beginning.

Step-by-step: prepare, sign, and submit the form

Follow these sequential steps to prepare, sign, and submit a Campaign Disclosure Form electronically or by paper.

  • 01
Prepare Document: Gather contribution, expenditure, and bank records.
  • 02
Complete Fields: Enter required data and verify formats.
  • 03
Sign & Authenticate: Sign, obtain notarization if required, capture audit trail.
  • 04
File with Agency: Submit via agency e‑file portal or mail.

How to configure an online filing workflow

Configure an e-filing workflow and conditional fields to match jurisdiction reporting rules before sending for signatures.

Field Configuration
Template Upload Upload PDF or DOCX and lock form fields.
Conditional Logic Show fields based on reporter type or thresholds.
Signer Authentication Choose email, SMS code, or KBA.
Auto-Export Save signed XML or PDF/A to records system.

Where and how disclosure forms are routed

Typical routing explains filing paths: federal, state, and local agencies and retaining a copy for your records.

  • Federal Filing: File with FEC online if threshold met.
  • State Filing: Submit to state election or ethics agency portal.
  • Local Reporting: County or municipal disclosure rules may apply.
  • Record Retention: Keep certified copy for the statutory retention period.

Digital delivery, formats, and integration considerations

Digital distribution options and integration considerations for submitting and storing Campaign Disclosure Forms electronically and securely.

  • File Formats: PDF, PDF/A, and DOCX accepted.
  • Integrations: CRM and cloud storage integrations available.
  • Authentication: Email, SMS, and advanced KBA options.

Common filing windows and schedule notes

Common filing deadlines, late-filing consequences, and periodic reporting windows vary by jurisdiction and office.

Federal Reporting:

FEC periodic and pre/post-election reports per FEC schedule.

State Deadlines:

State filing dates vary; check the state election office.

Quarterly Filings:

Many jurisdictions require regular quarterly or monthly filings.

Late Fees:

Penalties or fines may accrue for late or incomplete filings.

Amendments:

Amendments often have separate deadlines and must be marked accordingly.

Key milestones from preparation to public posting

Typical submission milestones from preparation through agency verification for a Campaign Disclosure Form and public record.

01

Data Collection

Assemble donor, expenditure, and bank records for the reporting period.

02

Draft Filing

Populate the form and cross-check amounts and entries.

03

Authentication

Sign electronically or obtain notarization if the jurisdiction requires it.

04

Agency Processing

Agency verifies completeness and posts the report to public records.

Common mistakes to avoid when preparing disclosures

  • Incomplete donor details cause rejections and trigger audit flags; ensure names, addresses, occupation, and employer fields are complete and reconciled to bank records to meet disclosure thresholds.
  • Incorrect date formats and mismatched reporting periods lead to misapplied contributions; consistently use MM/DD/YYYY and reconcile line items to the reporting period totals.
  • Failing to disclose in-kind contributions, aggregated small donations, or coordinated expenditures can result in fines; properly classify and value non-monetary support with supporting documentation.
  • Using unsigned or improperly authenticated signatures invalidates filings in some jurisdictions; verify ESIGN consent, local notarization, or witness requirements before submission.

Potential penalties and legal risks

Civil Penalties: Fines for late or incomplete filings
Criminal Liability: Intentional concealment may result in prosecution
Audit Risk: Increased likelihood of agency audit
Loss of Public Trust: Negative publicity and scrutiny
Disqualification: Possible ballot or office disqualification
Backup Withholding: TIN errors can trigger 24% withholding

Security and compliance measures for electronic filing

Encryption: TLS 1.2/1.3 in transit
At Rest: AES-256 encryption for stored data
Audit Trail: Timestamps, IP, and action logs
Access Controls: Role-based permissions and SSO
HIPAA BAA: BAA available when PHI present
Compliance: ESIGN, UETA, SOC 2 Type II

Fundamental components every disclosure should include

A complete Campaign Disclosure Form includes structured schedules, itemized contributions, expenditures, summary reconciliations, and signatory attestations required by the filing authority.

Cover Page

Identifies filer and reporting period; lists committee ID, contact information, and total receipts and expenditures. The cover page is used for indexing and public record searchability.

Contributions Schedule

Itemizes contributions with donor name, address, occupation, employer, date, and amount. Disclose aggregated contributions and identify those exceeding jurisdiction thresholds for full reporting.

Expenditures Schedule

Details payee, purpose, date, amount, and category. Include in-kind expenses and allocate shared costs across committees where required by agency guidance.

Summary Totals

Reconciles opening cash, total receipts, total disbursements, and closing cash on hand. Errors in totals commonly trigger agency inquiries and amendment requests.

Loans and Debts

Report loans, guarantors, repayment terms, and outstanding balances. Accurate debt disclosure affects committee solvency reporting and post-election reconciliations.

Certification & Signatures

Treasurer or authorized officer attests to accuracy, signs, and dates. Electronic signatures must meet ESIGN consent, attribution, and retention requirements for legal effect.

Real-world scenarios for submission and compliance

Two brief examples illustrate how offices and campaigns handle disclosure forms and maintain compliance when using electronic workflows.

Local Election Office

A county election office accepts electronic campaign disclosures and posts them to a public portal for transparency.

  • Requires notarization for certain filings.
  • Office procedures include automated field validation, an audit trail for each submission, and clear treasurer instructions for amendments to reduce processing time and improve public access.

Municipal Candidate

A mayoral candidate coordinates multiple reporting committees and uses e-filing to submit timely reports and reconcile donations across committees.

  • Detailed audit trail documents signer identity and timestamps.
  • Electronic submission, combined with standardized templates, helped the campaign avoid late-filing penalties during a high-volume reporting window and enabled rapid corrections when donor information changed.
