What the Candidate Selection Sheet Is and When It’s Used

A Candidate Selection Sheet is an internal HR document that records the hiring decision for a specific applicant, summarizes qualifications, and captures approvals and signatures needed to advance an offer. It consolidates candidate identifiers, scorecard results, interview notes, selection rationale, and required signatories so hiring teams have a single authoritative record. While it is not a public filing, the sheet supports downstream actions—background checks, offer letters, and onboarding—and must be completed accurately to meet recordkeeping, non-discrimination, and audit obligations under federal and state law.

Why a Clear Candidate Selection Sheet Matters

A well-structured Candidate Selection Sheet reduces ambiguity in hiring decisions, supports compliance with equal employment laws, and creates an auditable trail for HR and legal reviews.

Why a Clear Candidate Selection Sheet Matters

Primary Users and Stakeholders

Use role-based sign-off to ensure accountability; maintain a copy in HR systems and the personnel file after the candidate’s status is finalized.

  • Hiring managers and interview panels — prepare candidate evaluations, scorecards, and final recommendation for hire.
  • Recruiters and HR operations — verify candidate data, coordinate interviews, and attach background check authorizations.
  • Legal, compliance, and diversity officers — review for potential discrimination risks and ensure required approvals are obtained.

Step-by-Step: Complete and Approve the Selection Sheet

Follow these sequential steps to prepare, review, sign, and file the Candidate Selection Sheet for a hiring decision.

  • 01
Prepare the draft: Populate candidate details, scorecard, and interview notes accurately.
  • 02
Review internally: Hiring manager and HR verify qualifications and legal compliance.
  • 03
Obtain approvals: Secure signatures from required approvers in the documented order.
  • 04
File and archive: Store final sheet in HRIS and personnel file with audit trail.

Essential Elements to Include on a Professional Selection Sheet

A comprehensive sheet combines candidate identifiers, objective scoring, documented rationale, and a clear approval path so decisions are auditable and defensible.

Candidate Details

Name, candidate ID, contact details, and requisition number to match background checks, offer letters, and payroll setup.

Qualifications Summary

Concise summary of relevant experience, certifications, and minimum qualifications used to justify selection decisions.

Interview Scorecard

Numeric or rubric-based scores across competencies with weighted totals to allow objective comparison between finalists.

Selection Rationale

Clear explanation of why the candidate was chosen, including fit, skills, and business need, to support decisions if challenged.

Background & Clearance

Status of background checks, drug testing, or required clearances and any contingencies related to results.

Approvals Log

Names, roles, signature blocks and dates for hiring manager, HR, and any required executive or legal approvers.

Security and Compliance Basics for Handling Selection Sheets

Data encryption: TLS 1.2/1.3 in transit; AES-256 at rest.
Audit trail: Capture timestamps, IPs, and actions.
Access controls: Role-based permissions for HR systems.
HIPAA considerations: BAA required when PHI included.
Authentication: Use MFA for sensitive approvals.
Certifications: SOC 2 Type II and ISO 27001.

Common Risks When the Selection Sheet Is Incorrect or Incomplete

Noncompliance with ESIGN: Consent or attribution missing
Discrimination exposure: Unjustified selection rationale
Privacy breach: Unauthorized PHI disclosure
Background check delays: Incorrect candidate identifiers
Recordkeeping penalties: Failure to retain required records
Operational errors: Offer mismatch or onboarding delays

Typical Preparation Mistakes to Avoid

  • Leaving interview notes vague or subjective instead of factual and role-related.
  • Using inconsistent job titles or candidate IDs that prevent cross-referencing with HRIS.
  • Failing to document contingencies tied to background checks or verifications.
  • Skipping required approvals or using initials where full signatory evidence is required.

How a Candidate Selection Sheet Moves Through Your Workflow

This illustrates the typical path from draft to final archival, clarifying responsibilities at each step.

  • Draft completion: Recruiter compiles candidate data and scorecard.
  • Internal review: Hiring manager verifies evaluation and fit.
  • Sign-off: Approvers add signatures and dates.
  • Archive: Final document stored in HR record system.

Configuring an Electronic Approval Workflow

Define field behavior, signer order, and authentication to reduce signing friction and preserve legal intent.

Field Configuration
Signer order Sequential routing by role
Authentication Email or SMS code
Conditional fields Show based on selection status
Audit requirements Capture IP and timestamps

How to Share and Sign the Selection Sheet Electronically

Ensure the chosen platform records intent, attribution, and retains a tamper-evident audit trail consistent with ESIGN and UETA.

  • Secure email delivery: Encrypted link with access code
  • HRIS integration: Attach finalized sheet to candidate profile
  • eSignature platform: Integrations with systems like signNow
