What the Caregiver Employment Form Is

A Caregiver Employment Form documents the working relationship between a caregiver and an employer (family, agency, or household). It records key terms — duties, schedule, compensation, start date, tax and payroll classification, background-check consent, and confidentiality expectations — and creates a written record used for payroll, tax reporting, and compliance. When completed correctly it supports withholding and reporting, verifies identity for I-9 completion, and can be stored electronically or signed with legally recognized eSignatures under ESIGN and state UETA laws.

Why a Clear Employment Form Matters

A well‑crafted Caregiver Employment Form reduces misunderstandings about duties, pay, and hours, establishes payroll and tax status, and documents consent for checks and background screening. It creates evidence for labor compliance and simplifies onboarding and recordkeeping.

Why a Clear Employment Form Matters

Who Typically Completes This Form

The form is used by private families hiring in‑home caregivers, home care agencies onboarding staff, and payroll or HR teams documenting household employment.

  • Family Employers: Private households hiring full‑time or part‑time caregivers to document duties, schedule, pay, and benefits.
  • Home Care Agencies: Agency HR uses the form to capture assignment details, client requirements, and compliance authorizations.
  • Payroll / Bookkeepers: Professionals use completed forms to set up withholding, report wages, and prepare I-9 and tax returns.

Accurate completion by the hiring authority and the caregiver ensures payroll, tax, and legal steps proceed without delay.
