What a Certificate of Insurance Sample COI Shows

A Certificate of Insurance Sample COI is a templated example of the Certificate of Insurance document used to verify that a party maintains specified insurance coverage. It summarizes insurer name, policy numbers, coverage types, limits, effective and expiration dates, and the certificate holder. Sample COIs are used to confirm compliance with contract insurance requirements, to request additional insured endorsements, and to show proof of coverage during bid submissions, site access, or vendor onboarding.

Why a Sample COI Matters for Contracts and Compliance

A clear sample COI helps contracting parties confirm required limits and endorsements quickly, reducing administrative friction and clarifying expectations ahead of contract performance.

Why a Sample COI Matters for Contracts and Compliance

Who Typically Relies on a Certificate of Insurance Sample COI

Common users include procurement teams, general contractors, property managers, and event organizers who must verify insurance before work or access is approved.

  • Real estate managers verifying landlord and tenant liability coverage for leases and property access.
  • Construction contractors confirming subcontractor commercial general liability, workers' compensation, and additional insured endorsements.
  • Event and venue operators checking vendor liability limits and evidence of product liability or cancellation coverage.

The sample COI serves as a reference for insurers, brokers, and certificate holders to ensure the final certificate meets contract terms.

Standard Elements to Include in a Professional Certificate of Insurance Sample COI

A professional sample COI clearly labels each coverage line, shows per-occurrence and aggregate limits, lists policy numbers and effective dates, and names the certificate holder and additional insureds. Presenting these elements in a consistent layout reduces review time and supports contractual compliance.

Insurer

Name of the insurance company issuing the policy and contact details for verification and endorsements.

Policy Number

Unique identifier for each policy so reviewers can confirm coverage details with the insurer or broker.

Coverage Types

Specifies Lines such as General Liability, Auto Liability, Workers' Compensation, and Professional Liability.

Limits

Per-occurrence and aggregate limits for each coverage type, often expressed in standard dollar amounts.

Effective Dates

Policy start and expiration dates that determine whether coverage will be active during contract performance.

Certificate Holder

Name and address of the party asking for proof of insurance, often the contracting entity or venue owner.

Follow these steps to prepare a compliant COI that satisfies typical contract requirements.

  • 01
Gather Policy Details: Collect insurer, policy numbers, limits, and effective dates from the insured or broker.
  • 02

  • 03
Complete Certificate: Enter accurate names, addresses, coverage lines, and coverage limits on the COI template.
  • 04
Deliver to Holder: Send the COI to the certificate holder and retain a copy for contract records.

Typical COI Preparation and Delivery Workflow

  • Authentication: Email, SMS, or multi-factor signers
  • Integrations: CRM and cloud storage connections supported

Common Timing Expectations for Issuing a COI

Timely delivery of the COI prevents work stoppage, contract breach, and delays in payment or access approvals.

At Contract Award:

Provide COI before contract execution if required by the agreement

Project Start:

Certificate must be valid with effective dates covering project commencement

Renewal:

Supply updated COI on policy renewal or upon expiration

Upon Request:

Deliver within the time period specified in the contract or within reasonable commercial time

Claims Notice:

Make endorsements available immediately when a claim or incident arises

Frequent Errors When Preparing a COI Sample

  • Using an insured's DBA instead of the legal entity name, which causes verification delays with insurers.
  • Omitting required endorsements such as additional insured or waiver of subrogation specified in the contract.
  • Entering incorrect policy dates or limits, potentially exposing the certificate holder to uninsured risk.
  • Failing to attach the actual endorsement document when the COI indicates it is included.

Consequences of an Incorrect or Incomplete COI

Contract Delay: Work may be delayed if proof of coverage is inadequate
Financial Exposure: Certificate holder may face uncovered losses
Denial of Claim: Insurer may deny coverage for noncompliant activities
Breach of Contract: Supplier could be in breach for failing to meet insurance terms
Regulatory Risk: Industry regulators may impose penalties for noncompliance
Reputational Harm: Repeated COI issues can affect vendor standing
