What the Change of Address Name Form Is and when it's used

A Change of Address Name Form is a standardized document used to update official records when a person or organization changes their mailing address or the name associated with an address record. Typical use cases include updating postal forwarding, bank and utility accounts, employer records, and government agencies. The form collects identifying details for the party making the change, the previous and new name/address information, the effective date, and an authorization signature. Accurate completion helps ensure timely delivery of mail and correct legal and billing records.

Why maintaining an accurate address and name record matters

Keeping name and address records current reduces missed notices, billing errors, and identity verification problems with banks, government agencies, and service providers. A completed Change of Address Name Form creates an auditable record of the update and a clear effective date for downstream systems and legal notices.
