What a Change Request for AOR Is and when it applies

A Change Request for AOR (Agent of Record) is a signed instruction authorizing an insurer, plan administrator, or client to transfer representation and commission rights from one agent or broker to another for a specified policy or account. It documents the policy identifier, current agent, incoming agent, effective date, and the policyholder's authorization. Insurers use this form to update internal files, direct premium notices, and reassign commission payments; many carriers also require supporting documentation such as carrier-specific authorization forms, proof of appointment, or signed client authorization.

Why completing a clear Change Request for AOR matters

A precise, signed change request reduces processing delays, prevents commission disputes, and creates an auditable record that protects the policyholder and both agents. Using a standard form ensures carriers receive the information they require to update records accurately and to maintain continuity of service.

Why completing a clear Change Request for AOR matters

Who typically completes and signs this form

All signing parties should confirm identity, review policy numbers, and retain a copy of the executed request for their records.

  • Policyholders or authorized business officers initiating agent changes on their account.
  • Outgoing and incoming agents to confirm commission transfer and representation details.
  • Carrier or third-party administrator teams to validate and record the authorization.

Core components to include in a professional change request

A complete form contains defined sections so carriers can act quickly and maintain a clear audit trail.

Policy ID

Exact policy or account number as shown on carrier records; critical to avoid misapplied authorizations.

Current Agent

Full name and license number of the agent currently on record, plus agency name and contact details.

New Agent

Full name, license number, NPN if applicable, agency name, and contact information for the incoming agent.

Effective Date

Clear MM/DD/YYYY effective date for the change and whether it applies to new business, renewals, or both.

Authorization

Signed client authorization with printed name and capacity (owner, corporate officer) that transfers agency rights.

Supporting Docs

Carrier appointment proofs, agency contracts, corporate resolutions, or power of attorney attachments when required.

Common mistakes that delay AOR changes

  • Entering an incorrect policy number or insured name sends the request to the wrong record and requires manual correction by carrier staff.
  • Failing to include outgoing agent details or appointment proof leads carriers to hold or reject commission transfer requests.
  • Using ambiguous effective dates (e.g., 'immediately' without MM/DD/YYYY) causes uncertainty about billing and commission cutoffs.
  • Submitting unsigned forms or signatures from unauthorized signers triggers rejection and may require notarized or corporate-authority documentation.

Risks and potential consequences of an incorrect submission

Processing Delay: Lost time
Commission Dispute: Payment disagreements
Coverage Misrouting: Notices sent incorrectly
Rejection: Form returned unsigned
Privacy Exposure: Unauthorized data sharing
Regulatory Issue: Appointment violations possible

Practical tips for accurate and efficient submissions

Adopt consistent procedures to reduce errors and disputes when changing agent records.

Validate policy identifiers
Before submission, cross-check the policy number and named insured against the carrier’s declarations page or the policyholder’s documentation; mismatched identifiers are the single most common cause of rejection and require manual carrier intervention to correct.
Standardize templates
Use a carrier-specific template or a standardized internal template that includes all required fields, signature blocks, and checkboxes for appointment proof to reduce repetitive follow-ups and ensure consistent data capture across accounts.
Capture authority proof
When signing on behalf of an entity, attach a corporate resolution, power of attorney, or other proof of authority; carriers frequently request such documentation to validate that the signer may reassign agent representation.
Confirm receipt and closure
Request written acknowledgement or an updated declarations page from the carrier and reconcile commission statements for at least one billing cycle after the effective date to ensure correct routing and prompt investigation of any discrepancies.
